Well very large doses of MDMA do cause serotonin syndrome. The reason why most doses don't is because the body has many ways of clearing serotonin out of the brain. The serotonin transporters should still be largely functional at most doses of MDMA, though the amount of serotonin released by them initially outweights the amount they can take back. There are also enzymes around which break down MDMA...
Thats why if you take an MAOI and MDMA, you're really running the serotonin syndrome risk, because the brain has hardly any ways of getting rid of the serotonin.
