IV Sedation for wisdom teeth extraction

Hey, I just wanted to add some questions to this thread. I'm going to have surgery at the end of the month (rhinoplasty/septoplasty because I've broke my nose at least once and it's all f'd up, looks bad and I can't breathe through it). Anyway I'll be getting IV sedation or twilight anesthesia, and I'm curious what role tolerance plays in being sedated? I mean obviously it has an effect, I'm just curious of how strong an effect. I like this thread so far but I'd love some more stories of drug user's experiences with being sedated for any surgery.

I've mainly been an opiate user (on and off) for about 5-6 years. My current use is just 4mgs of bupe a day and the occasional morphine binge/cannabis smoking. I definitely plan to taper off my bupe (with kratom) a week or two before the surgery and have at least 5-7 days cold turkey. I'm just really nervous, I've never had surgery of this scale and the times I've been sedated it never seems like enough. And if someone is going to cutting open my face and shaving and breaking bones I'd really like to be ffffffucked up to the point where I don't remember a thing. Plus it takes quite a bit of morphine like 150mgs IV (over 2-3 shots) to make me nod and I'm thinking the standard dose on fent isn't going to cut it for me.

I'm going to be upfront with the anesthesiologist (mostly, at the same time I don't want to get skimped on post-op painkillers) and tell them I used to be scripted benzos for anxiety and in my past used opiates recreationally and also tell them about the lack of anesthesia I've felt previously. I'm paying 5k for the surgery so I assume they'll make me "comfortable" probably much more comfortable than someone just getting a tooth pulled. It's just my idea of comfortable will probably take more than the average guy. But I really hope since I'm personally paying for their services they might be a little more liberal, especially considering my circumstances.

last question, has anyone requested a certain medication for anesthesia or post op? Mainly I'm thinking of asking for ketamine for my main knockout drug with a dash of fentynal/benzo ...I've only got to use K a handful of times so tolerance isn't a problem and from my googling it seems like one of the safest anesthesia drugs (if you can handle the weirdness...which i most definitely can ). I think the fact that I'm paying them SO much money, they will be more likely to do what I ask (within their professional opinion or course). I'm probably not going to ask for anything stronger for post op pain, unless they give me 5/500 hydros or anything weaker...shit if 5/500 vicodins didn't cut it for my wisdom teeth I doubt they would work for this.

I just got my wisdoms out and they gave me IV valium.

During the surgery I could hear things like grinding and pliers but I couldn't associate it with pain or a visual or anything...that was weird

Oh yea and the valium wrecked my vein but I guess that happens to ~20% people (according to the doc)

Had 4 impacted wisdom teeth in extracted in 2000. Dental surgeon suggested I go with anaesthesiologists which I enthusiastically agreed to. She used what she called "balanced" anaesthesia which consisted of versed, demerol, and ketamine in combination. The first drug I got was versed which was wonderful- all my anxiety just drained away. Unfortunately the last thing I remember was N2O beind applied and turned on. Didn't get to experience the demerol rush or effects of K except I have a vague recollection of the surgical staff laughing ( at probably something I said while under), and seeing people in blue scrubs and face masks in what seemed like a fun house mirror. Next thing after that I remember trying to jump off the gurney in the recovery room and having my parents urge me to lay down. Wish she woulda been cool and given me the demerol first and then the ketamine followed by the other stuff?

I just got all 4 removed and they gave me a script without asking for 2 valium 10mgs to take pre-op, but they started my IV, I knew I was getting midazolam and fentanyl already, they said you'll start to get sleepy and after about 30 seconds they said I needed another 4mg of midazolam and that's all I remember. Mom told me afterwards the doctor said he used diprivan also, which I didn't know about. Nothing hurts or anything plus They gave me a script for 20 loricet 10/625s with 1 refill & I called and said it made me itch too much so they gave me a new one for percocet 10/650s with 1 refill

I got propofol/midazolam/fentanyl IV combo with lidocaine as a local I believe; Perc 10/325s afterwards with another shot of IV fentanyl to go. I just always overdo it alittle with how bad things hurt to make sure I get the most out of the experience

but all the surgeons knew I was a junky(it was kinda hard to hide when yer in a gown and theres track marks all over me)


i gotta get another back tooth cut out and thats what scares me is them seeing the marks down my arms do you think that will affect what they give me or would you think they would give you more cause the percs just ain't going to do the job this time

^ they will prob give you something for the procedure but i doubt they will give you anything to take home. maybe but if you tracks are as bad as you make em out to be then i doubt it

but when i got my wisdom teeth removed i got all four out, and while it hurt the day of n the day after. the pain the following days was truly minimal at all times except while trying to eat or if i got excited and slipped up with laughing hard or talking loud. So i ended up being able to save quite a bit of the 60 (20 first, then another 20x 1 refill )10/500 Hydros i got

but everyone is diff, and if you go in there without your dope/in mild WD it will prob hurt/be uncomfortable as shit

also keep in mind most doctors sedate and opiate the patient, not only for the patient sake, but a relaxed non moving/flinching person is somewhat easier/steadier to work on/with

Was wondering what I may expect from this.. has anyone gotten this procedure done before? I heard a common drug for the IV sedation is midazolam- is this a benzo with a high probability to make you forget the entire treatment itself?

I am very nervous as I have never been put out before, and am wondering if it is nothing to be afraid of. I have experienced with ' mainstream ' benzos before, alprazolam, diazepam, clonazepam, etc.

I do not understand how using such a drug could help the pain and agony in such a pocedure!

If I ever go into surgery I am telling them 100% about my tolerance! Fuck that, having surgery done on you while your awake but can't tell em,,, errr makes my skin crawl!

I had mine taken out about three years ago and when i asked the doc. what i was going to be given to knock me out he strongly REFUSED to tell me. I kind wonder what would of happened if i knew that i was allergic or something to what was given to me and had a bad reaction. Don't they have to tell you what they are giving you?

It must of been Versed or something similar cause I was told to count backwards from 10 and only got to about 3, then woke up all agitated and wobbly. I was only 18 at the time and didn't know about anything besides weed and shrooms. Because I got dry sockets where all four teeth were I could have gotten a refill on my 30 5/325 oxycodone, but sadly i chose not to.

^ I was in rehab with a woman who was prescribed 640mg of Oxycontin a day because she had numerous health issues with her back. She was taking all the pain medications and went to rehab. I watched this woman with a broken back withdraw from eight 80mg OC's daily and she made it through rehab successfully. She was in horrible pain, but dealt with it. And you blame relapsing on the pain from getting your wisdom teeth removed?

Everyone is different. Some people have to go to rehab multiple times before they clean up. Just because you watched some lady with a heavy addiction and a lot pain clean up doesn't mean everyone whose pain/addiction you judge to be less severe has to clean up to meet your standards.

i'm getting my wisdom teeth out next week. there was a pamphlet at the doc's office about all of the different anesthesia they offer. they had ketamine, fentanyl, nitrous oxide, and maybe some benzos but can't remember. i have somewhat of an oxy tolerance, so i'm hoping that i don't remember any of the surgeries

which one is the most recreational for during and right after the operation? a friend of mine told me that nitrous was pretty euphoric.

and what kind of dosages would i need for pain? i can take 90-120mg of oxy, depending on my r.o.a., so how much would be required to relieve pain approximately?

and is there anything else that the dr. would be able to figure out if say i needed more fentanyl than most people? he's a family friend, and i have a decent, but not super noticable track mark from when i donate plasma, and my vein on my left arm is slightly faint track marks from shooting, but i'm hoping it's going to fade by next week.

I had it done, they used propofol and i'm guessing versed. I know there was propofol involved because i saw the milky white substance and knew what it was. Before the IV meds they put me on nitrous and cranked it up real high to the point where it was almost too much to handle; I almost told her to turn it off because I was getting pretty light headed. I did have somewhat of an opiate tolerance at the time, but not a very large one. I was messing around with Norcos and the occasional Percocet. One week prior to the surgery I was given #20 Hydrocodone/APAP 10mg/650mg and on the day of the surgery I was given another #20 of the same generic Norcos. The place where I got this done was an oral surgery office and they filled the medications right at the office so i didn't have to go to the pharmacy which was nice. As far as pain, everybody is different, but I thought it was pretty damn painful and I had to take multiple norcos to get rid of it.

i just had 3 wisdom teeth pulled out 3 weeks ago, i didnt have a choice, my oral surgeon told me i had to be put under. i was so terrified, but really it wasnt that bad, they started the iv, and i think the first shot he put in was versed, after about a minute or 2 things started to get very.... wavy and fuzzy and i said woah lol and closed my eyes and i dont remember anything after that but apparently i was talking about being on clouds hahaa then i remember a nurse asking me if i could wake up and i seriously thought i just went to my friends house lol i even told the nurse that then i dont remember walking to the other room but i guess i did lol then they brought my boyfriend in and i was telling everyone how much i love them lol all in all it wasnt as bad as i thought it was gonna be, recovering was a whole other story, that night i puked 3 times, my boyfriend said it was from the drugs and stress, but after that i was ok, im pretty much healed now and so glad i got them all out at the same time

When I got mine pulled last year they told me if I chose to be sedated they were going to use propofol and fentanyl... I just stayed awake... it wasn't bad... only took like 25mins for all 4... then I went and fillled the dilaudid scrip they gave me... much much better

And I didn't relapse even tho I had a massive dilaudid habit up till 3 rears ago...

This was my experience, I was conscious but forgot immediately.I was given a script for 20 vicodins but truthfully ibuprofen was good enough for the pain.

Don't mention any opiate tolerance or other drug use besides prescribed meds.

Lol I was told when they woke me up the nurse wher laughin at me and shit because I wouldn't stay seated and kept saying "Man I'm FUCKED UP" an "wayd ya do ta me shiiittt" Who no's what else I told em haha. The had my ride pick me up in the back so I wouldn't make a seen. My dad said they were ver nice about my sillyness though, and on top of that they gave me the iv for free I was just gonna go with novacaine its an exxtra 700 otherwise. I remember laying down and they told me to pull up my sleeve and I just smiled. Doc said I would feel drousy in a minute and I woke up in my dads car after that lol.

I was totally wasted afterwards and was falling around everywhere like i was drunk, you will need someone with you to help you get home and you wont be driving if you do unless you want to get jailed

Im not sure how long it lasts cos when i had it done i went home and smoked a dooby which knocked me out for a further few hours lol

It is used like you say so you dont remember anything .. i came around for what felt like a milisecond but my mouth was numb too

I actually enjoyed the feeling after .. i think my mum hated it who was the one that was there to help me get home xD lmao

Interesting thread, when I had that abscess that had my whole face swollen a couple weeks and ago and needed emergency surgery and draining to get the tooth out and drain the abscess they were about to put me out. they asked me to sign the papers for it, and it said midazolam and fentynal. I thought it was funny and a bit dangerous to give a benzo/potent opioid mix that could possibly be fatal any other time, but "this" time it's juts anesthesia, so anyways I was about to sign the paper till I saw the prices. the work that was being done was to costs $300 something dollars and the anesthesia by itself was a $500 charge, so I was like Nah man just numb me up, lol... they did give me some laughing gas first... but it didn't matter after they numbed me it was a piece of cake anyways.

I have been wondering this for a while and i guess this is the time to ask while this thread is on the go ...

If i need any dental surgery will they still give me this method of sedation due to the fact i have had an opiate and benzo use on my medical notes and that im on methadone

or would i have to detox first then get the surgery or a different method of sedation

im in the UK if this makes a difference

Midazolam and fentanyl are incredibly safe for anesthesia when the patient is honest to their surgeon and anesthesiologist. Typically they have someone like a nurse watch your heart rate and BP the entire time so if you are going to start to overdose, they'll stop it before you get there.
