Ok I had an idea regarding fentanyl gel patches, and I want to hear some thoughts on it. Just humor me here. We all know that fentanyl has a very high bioavailability when administered transdermally. But one problem with the patches is that they take so long to kick in. So WHAT IF you just cut open a patch, squeezed out a little gel, and rubbed it on your skin? Would it work? Would it take effect quicker? Has anyone tried this?

Yes. But instead of on your skin, you can put it on the inside of your cheek, which is called buccal administration. Also you can put it under your tongue, which is sublingual administration. There are many threads regarding this here on bluelight, if you use the search engine you should find a lot of helpful information. I forget the exact figures off the top of my head but in my experience cheeking fentanyl has been much more effective. It comes on much quicker and I am pretty sure the bio-availability is higher as well.

Be careful though; If you do this, start with a SMALL pin head sized gob. Because when you wear it, it is releasing slowly over 3 days. Putting it in your mouth makes it release instantly, and will be MUCH more potent. Start with a pin head sized blob and if you aren't where you want to be like 45 minutes later, dose a little more. It is always smart to start small. You can always do more, but can't take it back once you do. You don't want to OD.

Many people also like to vaporize the gel off of tin foil. I have done this in the past, and it hits VERY fast and gives a rush of sorts.

Though in the spirit of harm reduction, it isn't the smartest thing to do, to break the time release of fentanyl patches since is is SUCH a potent opioid. It is VERY easy to take too much. Especially when smoking it, it is very easy to smoke a blob that is too big and fall out.

If you have a small opiate tolerance you should honestly just stick to wearing them. That way you can feel good for 3 days instead of blowing thru it in 6 hours.

I agree with Brokendownpalace when he says "Be careful though; If you do this, start with a SMALL pin head sized gob."

It is extremely easy to overdose on fentanyl when not administered transdermally. It is EXTREMELY potent, like brokendown said, and overdosing on opiates is not fun (I've done it before, escaped with my life thankfully)

PLEASE - be careful.

^ yes (89% and 50% respectively)

I've read lots of reports of people trying to speed up and increase absorption of the patches through utilizing heat and such but there is a reason why recreational users by and large prefer to stick a bit of the gel in their buccal area rather than drop a dot on their skin.

As a whole you may have noticed that transdermal application is a route very rarely used by recreational users (of any substance) and this is largely due to the slow speed of absorption and why this route has more use in tapering protocols such as with nicotine, clonidine for opioid withdrawal, etc.

I'll postface this by adding that I have only a small amount of experience using fentanyl bucally as well as intravenously in clinical settings and no experience using it transdermally or via any other route.
