Sorry if this is a duplicate but I cannot find the original post anywhere.
I got hurt in an accident May of last year. Herniated L4-L5, Herniated C5-C6 (with bone spur), Sacriatic joint injury and torn rotator cuff. Started me out on 5mg HC TID. Useless. 10mg HC TID not much difference. On to 10mg HC TID + Diazapam as a muscle relaxant. Starting to feel SOME relief but certainly no quality of life. I am a 42 year old male 6’2” 240lb very active but not as athletic as I once was. I am new to taking pain meds as I have refused them most of my life. I am over my Heman days and don’t mind taking something that’ll make me feel better. They tried switching me over to Oxycodone a couple months back but I did not do well with it. Side affects were sickness, insomnia, and somewhat lackadaisical. They put me back on the HC 10mg 5 times daily with the Diazapam and Gabepentin. My condolences to anyone taking Gabapentin. It mad me down right dumb. Couldn’t walk straight half the time and could barely hold a clear thought in my head. I went to my Dr. yesterday and laid it on the line to him. 12 months now with no progress. No surgery options, pain meds help somewhat but I am basically in a position where I have to medicate and constantly relax or pay the price. I explained my life long love of Kayaking, Billiards, Hiking, and working on classic cars and would like to be able to do some of that at least to some extent. He upped me to 6 HC’s daily now but said if I wanted we could try a drug called Opana. He said with it I would only need to take two daily which sounds VERY appealing to me as 6 of anything a day seems like quite a bit. The 10mg HC’s seem to work decently for 2 hours then away they go. My question is this. Has anyone had to make this same transition? If so did the Opana work out better for you? I am hoping someone who has made this same transition could give me some insight on what they’re experiences were. Thanks in advance!

It's a lot stronger. But it might be right for you. Especially since you've been taking so many hydros already, which really isn't good for your liver (I assume they have 325mg of APAP/Tylenol in them as well?).

Opana is a lot stronger, so there's a chance it'll make you sick like the oxy. But with a low-dose extended release pill you might do ok.

Have you tried any other ER medications? OxyContin? MSContin? Fentanyl patches? All are quite strong but taken in low doses it's just as much or less medication than you're already taking.

If your Doctor is simply proposing giving you a stronger IR medication to dose as needed, I don't think that's your answer. If you're in 24-hour pain you should have a daily regiment that works, not just IR meds.

Do you utilize cannabis at all to help with the pain or inflammation?

Yes, it is 10/325 Acet. Well, I have tried MS Contin a couple times as I have a friend they are prescribed to who told me to give them a try when I was having a rough moment. I took it once and it worked VERY well with the only draw back being it took so long to take affect and I felt I had been stabbed in the stomach with a knife. I tried it another time along side one of my HC's and it worked perfectly. I keep an 18-20 hour day. I have been like that most of my life so my med cycle isn't that of a normal person I'd guess. I usually take 2 of the HC's first thing in the morning to get going and just skip a dose throughout the day sometime. Two upon waking and usually another in 2-3 hours and I seem to somewhat stabilize and mobilize for awhile. When I took the 30mg MS Contin along with my HC once in the morning upon waking I really didn't need another dose of anything for 5-7 hours. I haven't tried cannabis for the issues as I have never really done well with it. Smoke, laugh, eat, headache, crash. That's how MJ usually affects me. It's been probably 20 years since I've had any. I have had someone else tell me the same thing in regards to being strickly on IR medications. The way things are these days I am mortified of actually "asking" for anything. I live in the highest script abuse county in our state and the Dr.'s are hella strict. Thanks for the response and any other info you can throw my way.

All that acetaminophen will eat holes in your stomach as well. Opana ER sounds like a much better option for you, though I'm a patient not a doctor. I've just had to have surgery for an ulcer because I ate too many hydro 5/500s. Just don't want to see someone else screwed up by too much of what is essentially just cut anyway. sorry for the terrible spelling blame puff.

No apology required! I REALLY appreciate the input. I think I may go ahead and tell him to go that route my next visit. It does sound as if it would suit me better than what I'm taking.

If you do get the opana, I would suggest not snorting them because you'll never be happy taking them a directed afterwards.
