To the OP, it sounds like your tolerance to codeine is pretty low, so this may not be a viable option, but one thing you could talk to your doctor about is going on a short suboxone taper plan. For 240mg of codeine though I think that's overkill.
Maybe try some NSAID's like ibuprofen or diclofenac for your pain, even the ibuprofen/codeine preparations (still doing CWE) would be better for you. They're not great for your stomach and can cause ulcers, but they're a lot better for your liver which would have taken a pounding due to your paracetamol intake.
The good thing is the liver is very resilient and can repair itself unless you've gone way too far.
