I understand where you're coming from; but many of your points are somewhat bogus. For starters, I'm willing to wager that I've got more scar tissue in my nose than my arms. Also, a proper, sterile, injection...performed by an experienced IV user is nothing like cutting yourself. I do have to concede that IV use is more prone to cross the line and become an act of masochism. But no one would call a nurse or doctor a sadist for IVing patients with drugs that could be administered orally. I can't see how you could say someone is a masochist because they choose to administer their drugs via IV even if another method is available. Also, when you use via IV, the harmful substance bypasses all the other organs it would normally have to be absorbed through...thereby saving your lungs, throat, mouth, sinuses, etc significant tissue damage. I've seen just as many, if not more people with fucked up faces, teeth, lips, skin, sores, etc that smoked their meth than people who shot their shards.
*Edit* Not sure if I missed the key word of "seems" because I was baked or if you added it after I posted my response...either way, I would be a moron to disagree...yes IV undoubtedly seems like the most destructive MOA.

Actually, I've read reports listing smoked meth and smoked crack as being more addictive (by a tiny margin) than IVed meth or IVed cocaine....though for some reason this was only true for the stims...IVed heroin was still listed as more addictive than any other MOA for H. My personal experience was that IV is not neccessarily more addictive than smoking but addictive in a fairly different way. It's too hard for me to describe...but IV didn't have the compulsive perpetual redosing (ex: smoking it till it's gone) like smoking was...but when the craving did hit you, fighting it was like trying to climb a cliff while tied to a volkswagen...
