Dentists:I am lucky that in Birmingham we have a wonderful teaching school for dentists so everyone gets top class treatment for free,even if you work.Also,they have everything there,including the expertise to give you either a general or sedation.I always got my dentist to refer me there and they would give me sedation(midazolam).Like Blondin,I have no veins-not from using,i was born that way.I had my veins scanned when I had my surgery back in August and the intensive care doc said they were just very small veins.But at the dental hospital,they just escorted me down to the surgical ward and got one of their anaethetists to put a canula in.They can do it to babies,so they could get me every time(this was after several visits and after they realised the regular dentists,even consultants,couldn't get a vein).When I had my surgery,they choked me out,by 4 people applying pressure to arteries/veins whatever until i passed out.then they put a line in my neck and i was out.while i was under,they dug around for a better vein and when i came round,they had removed the one from my neck and i had this other one for my drips.
Anyways,even though we have the wonderful dental clinic,a dentist in a new area sent me to some kinda big clinic.They couldn't get a vein the one day,so sent me home to come back on a different day.that time,they got one and work was done.I always said that if a tooth was bad,don't fill it,just pull it so it won't bother me again,eventually I just said to take em all out(uppers).Now I can't grind my teeth things are much better.
The point is that I didn't tell them i was on methadone for 2 reasons.I was,a bit like you,worried that giving me IV benzos,if they knew I was on a script,they may not have given it to me or given me much less.But that's only cos of the benzos,it shouldn't have any effect on any novocaine/local that they give you.The other reason was cos I had to have someone there to take me home and keep an eye on me,because of the sedation,and i didn't want them knowing i was on methadone.I have never had any doc give me pk's.I hear they used to dish out diconal.I have had pethidine twice,when i was having my kids and it was ace!But I had no tolerance to opiates as i was clean throughout all my pregnancies.As for pain for after the event,they will tell you to take paracetamol at regular intervals.not to wait for the pain then take it,just to take it every 4/8 hours-whatever it says on the pack.The only things dentists ever gave me were strong antibiotics as if you have an infection,there is a good chance the local will have little or no effect,so this could be the cause of the problem some of you seem to have had with local anaesthesia.
I wouldn't tell the dentist;but that's more about me being a devious lying junkie.you should be fine either way.
One thing I would disagree with is having a toot afterwards.you know how bad it is on your teeth.you have a raw gaping hole in your gum which might not be helped in any way by you having a blast and getting that shit sticking all in the hole.it's a post extraction infection waiting to happen.
