I'd have to agree with the DXM. And I too was quite surprised when I first used it.
Certain antihistamines are good for increasing the sedation, but be careful, because I think there's some that do the opposite with codeine exclusively.
I'd have to say, if there was a ceiling dose for codeine, your tolerance rising would also make the ceiling rise too. Just a thought.

As far as anti-histamines go, sedation or not they go a huge way towards countering the side effects of nausea (which codeine causes as much as any opiate) and itching (which codeine causes even more than any other opiate, with the exception of high dose IV morphine). The best one seems to be doxylamine succinate (brand name Restavit). Promethazine works too, but I read a study which claims that it slightly decreases the potency due to mildly inhibiting the CYP2D6 enzyme, so I swapped to restavit. I tried diphenhydramine and one other which I don't remember the name of, but neither of them worked at all. You only need a low dose, maybe 1/4 - 1/2 of a tablet, taken at the same time as the DXM.
