One pill, many drugs
single pill can kill, but as many have stated, it's generally due to interactions, or the person's behaviour, rather than MDMA on it's own. Even single pills of PMA or 4-MTA are not dangerous (except in the ways outlined above), it's when they get mixed with other pills that they become dangerous.
There is one incident I can think of, but it was a capsule, as opposed to a pill. My other half is an NHS toxicologist, and they had a warning circulated about capsules containing a blue powder, that had been found in the North of England. Some bright spark had thought they could increase the effects by adding 5mg of strychnine to the dose of MDMA. Two people had taken them, one died and one came close. There was going to be a regional alert because of this, but a few days after, the N. Yorkshire police busted someone who had 40+ capsules. Although they couldn't prove he had compounded them, there have been no other sightings, according to the NPIS.
