"Best" Opioid for Breakthrough Pain

dunno why i didn't think of oxymorphone for breakthrough, but that shit is the stuff. a 5mg suppository for a semi-tolerant user is a blissfully pain-free experience (in some cases).
 
Don't wish to sound smug, but dipipanone pisses all over oxycodone. I don't know quite why, but dipipanone has an abuse potential on a par with heroin, possibly greater if you talk to some people. I remember reading that given a choice, in a study carried out, that after having a dose of both ketobemidone and diamorphine, half of opiate users actually preferred ketobemidone.

I think negrogesic mentioned that he'd tried ketobemidone, so he could tell you about what it's like (the dipipanone thing is through personal experience)

One other drug worth mentioning is dextromoramide (Palfium). It's nearly up with diamorphine in terms of abuse potential except for one thing; IV administration can cause a massive fall in blood pressure, sometimes enough to be fatal (I've seen the non-fatal results of IV palfium. The person drained of all facial colour, passed out and began to turn blue. If his feet hadn't been raised above head height, I wouldn't like to think what might have been the outcome)

Yes, you are entirely correct, dipipanone takes a flaming shit on oxycodone (and oxymorphone, yes oxymorphone). Although i have only tried it IV in combination with cyclizine (diconal tablets), it was able to pierce my, at the time 240mg methadone tolerance. That really says something about the drug, as it was able to get me high despite the tolerance (although i had to use a number of pills). Few drugs accomplish the feat of breaking through the methadone tolerance. Another one of those fews, is as we mentioned, ketobemidone (ketogan), which is an amazing compound, and i had it in pure form (no anti-histamine like with dipipanone)......

I have taken thousands of shots of heroin, and though i love (hate) it dearly, ketobemidone really takes the cake here. It also has the ability to permeate the strong-hold that methadone has on your receptors. If i had to choose one drug of unlimited supply, it might very well be ketobemidone. The individual I was getting ketogan from europe (customs intercepted a package once, and just re-packaged it and sent to me anyways) has virtually dissapeared, and despite its rarity, the person was charging me a suprisingly decent fee for such a prize as ketogan. The dipipanone (diconal) tabs cost me more, plus the fedex for urgent airmail to saudi arabi is like $110 for a letter weight (cash) envelope, though its not cheap to even western europe... (i like fedex)

And lets not forget levorphanol, a very overlooked and under-rated drug. It is quite potent, and though the rush (IV) isnt particularly profound, it lasts quite a while. Plus, out of these three drugs, only levorphanol can be prescribed in the US.

Again, all of these substances i speak of are from the perspective of IV use. It would be waste not to IV such rare compounds......
 
I agree with mostly all of this thread thus far. We were talking about synthetic opiates in the ADD forum. The two drugs that I like are lefetamine and levorphanol. Competing with heroin is difficult but lets not get things out of perspective. When people say they prefer heroin to drugs such as fentanyl that is not to say that drugs like fentanyl are not substantially similar to heroin. The main problem with fentanyl to my knowledge is the unique nature of respiratory spasm that it causes. I think that most people who take heroin enjoy 'seeing things' as one of the side-effects. I believe that if this component of the high was removed then it would becoem alot less recreational.

While in certain aspects, fentanyl and heroin may be very similar the subjective "highs" seem to be quite different. I have not experience fentanyl but have more experience than I wish to have with H. H definitely last much longer compared to the short lived high of fentanyl. I can't really comment on any other differences as I've not tried fentanyl and I'll let the experienced provide that info.
 
Ive actually had some pretty seriously realistic hallucinations and even an OBE from injecting fentanyl. But, technically, heroin would probably cause more, given its a kappa agonist (amoung other things), whereas fentanyl is just a mu agonist. But i dont know if hallucinations is a major driving aspect in heroin usage.....
 
I have Arthritis of the spine (Ankylosing Spondilitis)...I've had it for over ten years. My doctor first put me on Vicodin which did nothing for my pain....so he put me on 30mgs of MS Contin 3x a day and 30mgs of MSir 4-5 times a day for breakthru pain. The MSir was great, but the MSContin wasn't to good for my pain. Finally, my Doc switched me to 40mgs of Oxycontin 3x a day and 30mgs of MSir 4-5 times a day for breakthru pain........no more pain.....a great combo......I'm in heaven!
 
That sounds to me like your living in poppy-field cloud land. Lol 'cept you musta been in some pretty tragic pain to get given all those meds. Sorry I didnt actually mean hallucinations was the driving force for users taking heroin. That may be the case for psychedelics but def not heroin. I musta been a little confused when I made that statement. Still heroin users 'seeing things' is however fun but in a fucked-up wacky sorta crazy way.

Yes, that seems to be a common tendency in your posts, Ronald. have you ever even tried heroin? no? so what are you basing all of your statements on??? how can you possibly comment on the subjective qualities of a drug you have never even tried? please stop posting nonsense.

I've never experienced "seeing things" while on heroin. I've even recently begun to experiment with the IV route...god I love it...definitely lives up to the hype. Despite not getting hallucinations, I do get extremely blurring vision and have difficult seeing anything that's not up close when I do a pretty good amount of heroin. So, two questions:

Does anyone else get this blurred vision/trouble seeing far when on heroin?

And what type of hallucination or maybe, what types of things are you seeing while on heroin? Just examples or general things...I'm assuming it's not like high dose LSD or shrooms (or other visual hallucinogens/psychadelics).

I'm prescribed Oxycontin, and for breakthrough I am prescribed Oxyfast. Before the doctor moved me to Oxyfast, I was given compounded 20/175 hydrocodone/ASA tabs.
 
Ok, so some are saying Smyth = Ronald and Ronald = idiot. I don't know Smyth or Ronald or whether or not they're the same person. Sorry if that's sounds dick to anyone who mentioned this stuff..it's not meant to, just saying that I don't know the situation there.

Any way, is this hallucination on heroin just complete bullshit or is there ANY truth to it?

More importantly, does anyone get those same vision effects that I mentioned (blurred vision, especially hard to see things at a distance)?
 
I have had very vivid hallucinations while under the influence of large doses of heroin; sitting alone in a room, yet I talk to people who arn't even there. I see people walking in and out of the room...I would almost compare it *very loosely* to the psychosis diphenhydramine provides.

This occurs during a heavy nod when your in dreamland...
 
It's the hypogogic state maybe. That area between awake and asleep. Even people not on opiates experience it to a small degree while falling asleep, or while waking up (that'd be hypnopompic though)
