The most disturbing film you have seen?

I don't know how you could think that movie is real when the acting is just above that of a middle-school drama club.
 
I have to agree with everyone who mentioned Henry: Portrait of a Serial Killer.
It's one of the most disturbing films I've ever seen...

movie wise "The Life and Death of a Porno Gang" was sort of disturbing but more so depressing when u can actually relate to certain characters suicide.

once i saw a movie about this tribe of warriors that roamed around and did stuff. disturbing stuff. they kidnap this virgin princess from some village or castle. it's pretty romantic. but i don't know the title.

---Cold Fish (Jap movie about a dude who gets a new business partner in the fish trading business. Yes, you read that right, the fish trading business. He turns out to be a serial killer who forces him into the same lifestyle. English subtitles are around. Look for a torrent.)
Cold Fish is a good example of a disturbing movie that expands the genre instead of just piling on disturbing themes ("Let's make a movie with a cannibal priest who rapes autistic bulimic trannies with vomit fetishes and market the result as "uncompromising"!). Parts of Cold Fish reminded me of Visitor Q and I Saw the Devil. It's fairly unique among the titles listed here because it's funny -- and fully intends to be -- yet the comedy is so psychotically bleak it does nothing to divert from the multifaceted psychological and visceral assault it's making on the viewer. That's certainly a hard balance to maintain. It's difficult to know how "good" it was because I was far from sober while watching it, and I sure as hell don't endorse it's suicide-inducing message, but I remember thinking it disturbed in unconventional and well-planned ways. Certainly, it had strong technical merits.

and not that its gory, but that Buried movie with ryan reynolds, just because its so sad that he dies at the end. made me cry.
Ichi the Killer was pretty bueno
 
In terms of disgusting disturbing theres things like August Underground, Guinea Pig series etc, but I'd hardly classify them as movies, just senseless gore take n to a new level for the sake of it.
Otherwise, Visitor Q...Im trying to think, I know theres a couple which really got to me that I can't remember. Lady Vengeance has a pretty fucked up premise. Although I haven't seen all of these two, only parts, Salo (120 days of sodom) and Men Behind the Sun aren't pleasant. For some reason I couldnt handle 21 grams while I was tripping and I just had to leave the room, not to say thats by any stretch the most disturbing movie.

For some reason I had been hearing a lot about Event Horizon as a good psychological horror movie...

It most certainly is not. My first clue should have been that it was directed by Paul W.S. Anderson of the Resident Evil franchise. I kept waiting for it to get good but nothing ever happened.
