Your most embarrassing drug-related moment

^^You stingy bastards, that wouldn't fuckin touch the sides...

Serious question though, as a long term opiate addict, in the event of the above scenario what would actually be given to me in the way of pain relief? Presuming I was able to tell you that I've been an opiate addict for 20 odd years, would you up the doses accordingly or just tell me to 'MTFU you junkie bastard'?

This is something which has worried me for a long time...

Yeah sure...just drive your car into the back of a lorry...mangle your leg into a bloody 90 degree mess with the bone sticking out...dial 999 and we'll be there in 8 minutes in a nice Mercedes Sprinter with a bag full of drugs.......admittedly its a bit of an effort for 20mg of morphine and 10mg Ketamine but hey....each to their own :-)

Out of interest what sort of dose of Ketamine do you give someone when there limbs are mangled? Is it knock out dose? How many MGs?

I'm glad you found it funny, I'm sure my antics that day robbed a fair few yoots of their innocence.

"Mummy - why was that man's eyes so big and why did he keep asking me for cigarettes?" =D

Reminds me a bit of when I was staggering back to halls from halfway across Birmingham on my last night out up there. I had to get the first train of the day, I was completely off my nut on coke and meph still after being on it for about 12 hours by that point, and was dressed in a very small crop top and hotpants combo (was planning on only going clubbing, not walking miles in the early morning chill).

Well if that wasn't shameful enough when I got on the train the first thing I did was head for the toilet - which was opposite this sweet looking woman and her cute little girl who had all their suitcases like they were off on holiday or something. After a brief struggle I thought the toilet was locked so I gave up and, disheartened, returned to my seat only for the little girl to tap me on the shoulder seconds later to tell me they'd managed to open the toilet for me. I thanked her, and feeling vaguely ashamed rushed into the cubicle - where, of course, I proceeded to rail a huge line of coke off the conveniently placed window ledge. When I came out she smiled at me as well, I felt kind of guilty lol. At least she was young enough to have no idea what I might've been doing :P

:D

Racking up on a train has lost all its appeal since losing the contents of a wrap to the pissy floor :!

Sounds like a great night (I mean morning) to me. Surprised you managed to feel even 'vaguely' ashamed on coke. ;)

We're not allowed to go over 20mg of morphine for anyone...regardless of tolerance... We would usually start at 5-10mg and titrate up based on your pain scale but legally can't go over 20mg. We would supplement with entanox (nitrous) and if your really, really fucked then ketamine but not all paramedics are allowed to admi sister ketamine only team leaders and HART (hazardous area response teams). That combo may not be ideal but should keep you somewhat comfortable on you way to hospital....entanox is stronger than most people think....I've seen people screaming their heads off who calm right down after a few puffs of the old nitrous. I know if you have a sizable habit 20mg may seem fuckall but more than that could kill an opjopiate nieve person and we have to cover our backs.......we would never call you or anyone else a junkie though. Most paramedics are compassionate people who are genuinely their cos they want to help you.......we certainly arnt in it for the (shit) money!!!

Thanks! Good answer, and somewhat reassuring :) Are you saying though that paramedics CAN administer morphine, but not ketamine? If so, how come seeing as ketamine has a much better safety profile than morphine?

Oh, BTW, I know first hand how good paramedics are - I have them to thank for still being here now, so hat's off to you if you're a paramedic, you do a fantastic job!! :):)

Don't ever change summer. <3

FUBAR......Glad we could help and glad your still here mate!!!....as your probably aware we carry Narcan as well!!!

As far as I know, I didn't get a shot of naloxone until I got into hospital (I woke up with it still in my arm), but you guys got my heart started twice in the ambulance, for which I am eternally grateful <3

At the risk of derailing this thread any further, what happens when you come across someone with severe physical trauma and you cant find a vein due to past drug addiction. Is there a site where you're almost guaranteed to get an IV line in? This is something else which worries me about my past excesses :(

Sorry to everyone for derailing the thread but this is obviously something your concerned about and I don't want you to worry about it. After the obvious sites I.e. back of hand, brachial vein in arm (main line), foot etc....we would probably try the jugular. If all that fails then it depends..... We can give either IM morphine or oramorph or if its absolutely vital to gain IV access immediatly to admi later fluids if someone has lost a rediculous amount of blood their is a procedure called an venous cut down where we can cut into your arm to find an internal vein but this is hardly ever used...then of course there is inserting a line directly into the bone marrow of your hip with a drill but the last two are very, very rare and I've o my seen them done once and it was by a doctor. Usually we can get a line into your jugular if your ar, hand and foot veins are fucked though and as I say there is always IM morphine (which we've only recently been allowed to do for some reason ) and oramorph. The other procedures are usually reserved for people with more training than a basic paramedic. I wouldn't worry though.......we're very, very good at gaining IV access in poor veins......their are lots of trickz which I won't detail this thread further by going into. Whatever happens we'll look after you...don't worry mate

Thank you! I'd glad to say my jugular is still nice 'n' juicy =D

(please carry on OT now folks..)

Last time I had to have blood taken I had to show the phlegbotomist where to go after him insisting he would be able to get blood out of veins that were visible but I knew were collapsed. I hate to think what it would be like now, it would have to be soles of the feet most likely or femoral vein (if you ever risk that).

It can never be as bad as having blood taken from an artery, that sucked (unless you're being drilled out or something).

I have to get my blood taken pon da regs - it takes them forever to find any. They usually get it from inside my elbow after a few tries, but had to have it taken from my hand a couple of times and it left a whopping bruise both times.. Never injected anything - not really cut out for it by the looks of it.

Never use the femoral vein...too close to femeral artery and femeral nerve. Never go for arterial access either as that's for sampling arterial blood for blood gasses. Some hospital plegbotimists are pretty shit although using one of those vacuum devices for sampling blood is a different skill than inserting an IV cannular. To be truthful no one knows their own veins better than someone who has repeatedly sought then out for IV drug use and really medical professionals should listen to them if they say that you can't access a particular vein (but seldom do). IME phlegbotimists often make the error of removing the tourniquet too soon, not looking for valve location etc.

In the interests of harm reduction. There are some online courses in venopuncture that only cost a few quid and are very informative and teach comprehensive and professional IV access. Charles Bloe is the favoured online training provider for the ambulance service. Maybe check it out....could save future vein damage.
