Who could I see about trying naltrexone to reverse opiate tolerance?

I have a pain management doctor who prescribes me opanas, 40mg 2x a day and 10mg ir 4x a day, and I thought he might be a good place to start. I'm just so sick of needing so much opiates to relieve my pain and being so dependent on these opiates. A little history about myself, I got an infection in my spine a little over a year ago and it got to the point where I could barely walk before I got it treated. I believe it may have come about from IV suboxone but I'm not sure. I ended up in rehab right as the pain started developing, and had to leave rehab because they wouldn't take me to the hospital to find out what was wrong, and i was in 10/10 pain over half of the day. I could barely get out of bed to go get aspirin from the nurse. So I left and then ended up in the hospital. I told them I was less than 2 weeks out of rehab, and for some reason not 20 minutes later they offered me IV morphine, and stupidly I took it. Then later that day they bumped it up to IV dilaudid. I ended up leaving the hospital a week later with like 60 opana 40s and 200 roxy 30s, less than a month out of rehab. I have since relapsed ;to heroin as well, and although my Dr doesn't know I've been using (I get clean for my DT's every month), I've come to the point where I can't get high even off of a gram of IV dope all at once, and I barely get pain relief from my pills. I want to get to the point where if I run out 1 day early, I'm not glued to my bed sweating my ass off until I get my refill. I want to get to the point where I can one day quit everything all together. My goal isn't so much to be able to get high again, though it would be nice, but every time I read something about how naltrexone can reverse opiate tolerance, I become so intrigued. So my question is, who can I see about this? I doubt my Dr is going to keep me on these pills forever, as I'm only 23 yrs old, and I fear that him taking me off them would leave me with such a bad habit that I would start stealing and really hurt the ppl around me. Any help is appreciated.
