Opioids tramadol losing its effect

I am on tramadol again for long term pain as I can't tolerate opiates due to the GI problems they give me.

Since going back to tramadol it doesn't seem to work properly. I am supposed to take 50mg capsule as and when I need to, but it isn't doing anything other than making me, reallly, really, tired. I remember in the past tramadol gave me a new life; it made me more energized, comfortable, and actually able to do things. Now, tramadol just makes me crash. I feel depressed, tired, unmotivated, and in pain.

Does anyone know why tramadol would work before but not now? They are the same pills exactly - same brand etc.

Is there anything I can do to make it work better as well?

I'm not sure about why, and granted 1 of them makes you tired, I have to take 100mg QID (and really need more than that). Have you tried seeing what taking 2 of them would do for your pain?

I tried taking two 50mg pills at once one time to see if that would help, and all that seemed to do was potentiate the tiredness, lethargy, and depression while not doing anything more for the pain. My eyes and head just become so tired after taking them, like I need to go for a nap, but I am in too much discomfort to be able to do that despite being very tired.

Does anyone know if this is cause by a specific chemical in tramadol that is not agreeing with me?
 
If you aren't on other meds, and the tramadol is making you feel this way, I wouldn't suggest to keep taking it. Why don't you try a low dose opiate with APAP, if you get GI problems from them. Something as simple as a Vicodin every 4 - 6 hrs. could be the answer.
 
It depends on your pain level. Tramadol is a complex drug, that requires conversion by the P450 enyzmes, into an active metabolite that is an opioid agonist. Tramadol itself has almost no affinity for the receptor, so this conversion is necessary for it to be an effective analgesic.

The reason I am saying this is because that aspect, as well as the nature of tramadol itself, make it an unreliable drug. Tramadol was actually one of the first opioid I tried, and I loved it the first time I took it, but now I can't stand them. Granted, my tolerance is too high to take them anyway, but even with a lowered tolerance or in conjunction with other meds, it has the same effect on me as you described, just the various adverse effects with no distinguishable opioid effects whatsoever.

It is imperative you switch meds, if it is having this effect on you. Depending on your pain level, low dose hydrocodone could work, though if tramadol isn't cutting it, I think percocet would be a better option, because hydrocodone is also a prodrug, requiring the same(?) enyzmes to convert. As far as the GI problems, low dose opiates shouldn't cause any problem, because if the Tramadol was doing like it is supposed to, it would have the same effects on your body anyway. If you take low doses, and it is a problem, then you need to speak to your doctor to find out why, and if any treatments exist.

If your pain is severe enough, you can always try alternate methods of administration, such as snorting and rectal, though this is a slippery slope, and only suggested if your pain is at least moderate. Also, anything with tylenol isn't suitable for snorting, and finally, DO NOT take tramdol any way other than by mouth, because otherwise almost none gets converted to it's active metabolite, getting you only the serotogenic effects.
 
What i would do is go int that docs office and tell him he can take back all the ultram (tramadol)!!! It has might as well be in the grocery store next to the xtra strength tylenol??? But anyways Hyrdocodone is an ok suggestion but just watch the habitual aspect of them! they will jump and grab ahold of ya befor you know it but if i was to start go with the 10/500 its an average obviously popular strength of the drug, but that should do. Sorry for takin you around the world with this post but i cant stress on how habit formin these are!!!! NO JOKE good luck.....................peace
