Opioids Tough situation -- could use some other peoples opinions

I have been on this board for years and there are some really savvy people on here and I would like your opinions on something.....


So i have come to find out that my little sister (shes 19 i'm 28, so not so little) but i have come to find out that she has been abusing pills, namely oxycodone and oxymporphone. She has been using every day for about 4 months and has told me that at her highest she was using 2 40mg opanas. I was into pills in the beginning but moved into dope so im not as up on the pills these days but it seems that opana is everywhere now since Perdue has changed their Oxycontin formulation to be more abuse proof.

So here is the situation. She is on hour 36 without anything and its her first kick. My parents want to keep her in the house and let her kick cold turkey, but with the amounts she has been doing, i think she should be medically detoxed.

How is the WD from opana compared to oxy>? I would image it is pretty similar. I was already waaay too into dope when opanas came out so i never messed with them that much.

Also, for those who do use opana, is 2 40mg pills a day a huge habit? She was doing them intranasally so i am thinking its about the same as her doing 2 80's a day dose wise.

If it was me i would want a medical detox but its not my call on this one. So what do you guys think? 4 month daily habit that went all the way up to 2 opana 40mg's a day. Medical detox or let her ride it out cold turkey ish with OTC stuff?

I feel terrible for my parents for having to go through this again with their other child and i want to make sure my sister makes this first kick her last and doesnt turn out like me and get into shooting dope and being addicted for close to a decade.

According to Endo's conversion calculator 80mg of Opana is about 160 mg of oxycodone, orally.

IMO, that's roughly borderline of where I'd recommend OST or some sort of quick detox taper.


What she has going for her is that it's her first detox, so it won't be as bad as if it were her third or fourth. At least that's how it went for me.


Cold turkey will be a bitch, but certainly doable. And certainly the quickest way out.

A must have is loperamide, and perhaps some diphenhydramine, which may be helpful for sleep (not really), and some diphenhydrinate, which may be useful for the nausea/vomiting.


If I were you I'd look at her lifestyle, and see where that stands. Was she just getting high all the time? Was she doing shady things to support her habit? Is she in school? Does she work?

Depending how stable her life is, and if it's not very stable, you might want to get her into a Suboxone program. That should give her the support to get thinks on track.

As for the withdrawal difference between the more synthetic opiates and dope, I've always found the more synthetic opiates to have much worse muscle/joint pain as well as some stomach pain, along with the typical dope withdrawal symptoms of lethargy, diarrhea, vomitting, chills, sweating, etc.

I agree that the 1st will not be as bad, because the mind doesn't really know what to expect, and still fresh. I was also faced with such a dilemma one of those days and it was dealt with using some Valium and a few days with the family, man i wish i was so lucky! Anyways, good luck and listen to the above post. Different levels require different actions.
 
my first kick cold turkey was hell so im not sure why the others are saying that it was easier than others, but its different for everyone i suppose.



i dont think a medical detox is necessary, in fact i would encourage a cold turkey detox to show her what its like to live this lifestyle maybe the sickness will be enough for her to quit abusing pills

I've kicked off Opana several times. I find that it's really rapid, but not that long lasting. Usually about 36-48 hours of hell, then you start feeling straight.
I have gone from about 60-80mgs of Opana insufflated (that's equal to like 300-400mg of IV morphine) to none without needing a medical detox.

Opana is 10x stronger than morphine/ OC in absolute bioavailability. The BA snorted is 43%, so 80mg/day would be like 320mg of IV (Absolute BA) morphine or oxycodone, etc... Compared to H i would say it is 2x as strong (snorted vs IV/ mg) so that would be like doing one and a half PURE bags a day, or more like 15 normal bags.

She has a big habit, but like above people said, you need to figure out where she is at, if she is functional, and if when she goes back to regular life will her friends be bad influences or be supportive?
 
I have never in my life experience withdrawal, so I have very little to contribute here.

I just wanted to say that all little sisters should have a concerned older sibling like OP, talking about whether or not a medical detox is best, wanting his sister to stay away from dope, etc. etc.

I remember my older sister telling me that she felt bad about ever getting me into pain killers... and then later that night she asked me whether or not I wanted to try smoking crack with her. Yeah, thanks a lot, sis!
 
Thats a tough one because seeing your younger sister in that pain is hard but maybe thats what she needs to "feel" what these drugs will do to you. Medical detox is always better for the one involved though I've found.

My family had tried to medically detox me from 320 mgs of OC each day when I was 15, (im 22 now) I found that it just made me angry. Did she come clean herself or was she caught? If she was caught then I suggest a medical detox because the detox will feel less of a punishment and more of a loving gesture. If she came clean then she knew she would be in for some pain and I think that it really is best that she feel the pain of detox not as a punishment but as a reminder of how vicious these drugs are.

Make sure she knows she has the support of her family and that you are not angry but concerned. And that you want her to get better so she can be a little sister again.

Good Luck, and there are a lot of great support groups for younger addicts that have great time-consuming things to do with fun people if she needs a lifestyle change.. just google teen addiction support or meetings. <3

I'm currently using opana recreationally and I can tell u it's not hard to get up to 80mgs a day because tolerance builds like a ma fucker with those, I also need 60-80mgs to even think about catching a nod.

But i also live with my parents and they don't know anything I do and I have detoxed a couple times off opana and even more off oxy without them becoming aware of anything.
I've also had subs tho both times with the opana but if ur parents know she's detoxing then IMO she doesn't need medical attention, just make sure she stays hydrated and takes the general OTC medication that helps with withdrawal and I believe she'll be fine in a couple of days. But also as u already know I'm sure, if she starts reacting badly to the withdrawal then take her to the 'ER/ some professional.

What if you're taking these pills for pain management & you get hooked like most, is it still in the same scenario? I mean we all know what these pain pills can do but living with pain all my life isnt an option.

I totally agree here. im in the same boat.
 
She's over 18, so it's her decision to make. You and your parents can both present your sides and then let her decide which way she wants to do it.

At those doses, medically supervised detox would probably be the best option, but as others have said, cold turkey would be hell but would be the quickest way out.

There would of course be the issues with cravings and depression after cold turkey WDs are done, that's another thing to consider, something methadone or suboxone tapering can help out with.

There certainly are options for your sister, we wish her luck!
 
I agree with the idea that the more painful and horrific the detox is the more it will detur her from using in the future.

If she gets checked into the hospital where family and friends and nurses are all patting her on the head and saying "you poor girl, everything will be alright," she a lot more likely to think "well that wasn't so bad." She'll go through life thinking the worst case scenario for drug use isn't really so bad.

You have an opportunity here for a textbook case of tough love, and I think you should take it. Make this a terrible experience she'll never forget and never want repeated.

I don't agree with this. A couple of months after you detox, even if it's hell, it's all just a faint memory. There are people on here that have gone through countless horrible withdrawals and won't think twice about using again... People will use even when they're fully aware of all the potential consequences.

I see this kinda like the people saying "just lock up all the addicts, they won't use again." Doesn't usually work that way. :|
