If one has a fairly high tolerance to opiates and takes a break, anywhere from 30-60days+ and decided to take opiates again, where would it be safe to begin?

If someone doses 160mg oxycodone in instant release once a day for example (well not everyday but enough), always orally, what would be a reasonable starting point after such a break?

Any input would be appreciated. Thanks.

A 30-60 day break from opioid use, should nearly "reset" your tolerance. I'm assuming you plan on taking oxycodone. I would start with a 30mg dose, wait 1-2 hours to see how you feel, if need be re-dose another 15mg. So after your initial 30mg dose, dose in 15mg increments waiting at least 1-2 hours before dosing again. However, given that your tolerance will be nearly zero, 30mg should do the trick!

If you've been clean for that many days I'd go with a low dose. Much lower than 160mg that's asking for trouble. I'd say take 30mgs intra-nasally and see how you feel and adjust dose accordingly.

Start small. The best advice is to wait a few days, ask yourself if you want to go through that hell again.

Youll find your pschological tolerance hasnt lowered. Expect to be at your old tolerance within a week tops.

Expect tolerance to come right back to previous levels very quickly, within the week if you take up using daily again. Old habits die hard and previous opioid users are pretty much damned to fall into the same pits they've been pulled out of, over and over.

The best option of course is to never again bother with opioids and find some other way to spend your time.

If you're going to do it anyway, titrating up from nothing is how I'd go. Don't make any assumptions, start at something like 30mg of oxycodone and adjust accordingly.

I quit doing heroin after being addicted for 3 years and when me and my ex-gf got back into old habits we started with smoking a point between the two of us and that would be good for us. But, being former addicts we ascended in dose rapidly and within 3 months we were each shooting a p to get a good high. And then downwards from there. Sometimes new people and new living situation can't even stop a past addiction from rearing its ugly head again. Luckily we broke up and now that that dead weight is cut from my foot I've finally been able to be clean; even if it has only been for 3 weeks. I do occasionally use dilaudid to help with insomnia but it's only 4mg at a time at most. So take it for what it's worth, I'd stay away, it's not worth it. If I could re-do this whole heroin addiction thing for the last almost 4 years though I'd do it again, for me the ups outweigh the downs, but I really could have done without the last 4 years of my life. So frustrating!
