Bupe Switching from Methadone to Suboxone

Hey what's up? I recently completed this transition for the third time, every time being a complete success (no precipitated withdrawals, very little time spent actually sick).

Just for some brief background this time I has only been on methadone for about 3 months. The highest dose I got to was 35mg but I was steadily at 30mg when I discontinued dosing to make the switch. The whole time I was on methadone I was also using about a half gram or heroin IV most days.

Due to me moving and the clinic caring more about policy and money than me as a person I was forced to discontinue MMT in favor of suboxone. They had wanted $369 for the "intake" and wanted me to take off today and tomorrow from work to respectively complete my intake forms and then see the Dr, all the while knowing that most of my time spent at the clinic on these days would be spent sitting and waiting.

I took my last dose on Thursday, went to work, then used about 1gram of heroin IV after. The next day I went to work-still felt great-and used about .5g IV heroin after work. Saturday was the first day I began to feel withdrawals while at work. I was able to tough it out and made it to cop .5g heroin to use IV. I woke up Sunday morning sick as a dog, but still only a 12-13 on the Clinical Opioid Withdrawal Scale (COWS). My goal was to wait until I was bad enough and make the switch, but, my roommate being the nice guy he is came home and saw me like this and went out and got me a gram of heroin to split with him. We did it and I was ok for about 10 hours.

At around 9pm Sunday I began to experience withdrawals and as the night went on it got steadily worse. I have experience making the switch and I know my body pretty well so I was able to make a smooth transition, albeit not lacking a scare that I had sent myself into precipitated withdrawal.

At about 3am after sweating, yawning, sneezing, and fidgeting the hours away I crushed and took roughly 1mg suboxone IV (suboxone's bioavailability IV is around 98-100% compared to 30% sublingually. no the naloxone isn't going get in you and wreak havoc). Well I felt somewhat instant relief when injected followed by intense chills, sweats, cramps, and other withdrawals. I would go from withdrawing horrible to feeling warm and comfortable. This lasted about an hour until I just fell asleep. I slept only about 3 hours but I slept like a baby and woke up feeling great. I sit here now 9am Monday morning having injected another 1mg of subuxone upon waking and feeling awesome. My body has to get used the lower activity on my mu receptors and until then I WILL experience MINOR withdrawal symptoms such as chills (which is all I am experiencing right now). I figure by the time I wake up tomorrow morning I will feel great and can begin a 2 week long suboxone taper which has in the past gotten me off of everything without experiencing ANY withdrawals. It hasn't worked every time, but when it has it was great.

Have you switched from methadone to suboxone? What was your experience like? My uncle made the switch last week and it took him over a week to feel the way I do now. Different doses, different length on the clinic, different metabolisms. How has it worked out for you or swiy?

By definition BA of IV is always 100%

Methadone to sub is a bitch cuz methadone lingers in your system for days and is very easy to think you're sick enough, and proceed to get precipitated and get even sicker.

Also, bupe may not cover withdrawal completely if on a high enough dose of methadone, because of methasdone being so powerful and having no cieling, while bupe is only a partial agonist and has an incredibly low cieiling

I was forced to discontinue methadone at 85mg, after 18 months of steady use. Suboxone couldn't even put a dent in it, even after 7 days post-methadone..Horrible, HORRIBLE experience.. I ended up going back to heroin after about a month because I was still sick and suboxone did NOT help.

Suboxone normally works pretty well for me when coming off of heroin/other opiods, but that particular time coming off of methadone was HORRIBLE!

It's really weird, but an acquaintance of mine missed a dose at the clinic (they were on 50mg) and because they are not knowledgable in regards to pharmacology they figured they would take an 8mg suboxone strip. Surprisingly they didn't go into precipitated withdrawal, and told me they actually felt very similar to how they usually do when they take their 'done. I wish I knew for certain what Bupe's actual peak agonism is equivalent,as I would switch back to suboxone immediately once I was at the appropriate dose.

I'm incredibly glad to be off methadone.. Hah. If I were on it again I'd do whatever it took to get over to bupe.. though I know it isn't easy. I kicked methadone by getting back on smack... :\ That shit is brutal.

suboxone is safer than methadone, while maintaining efficacy and power to block other opiates. it make take large doses at the start if you are switching from a major habit, but it is easy and quick to go from 32 to 8 mgs. once you get at 8mgs, it starts to take longer to taper down mg per mg. many users on BL had extremely large opiate habits, and were able to taper down to 1-2mg a day, which is dose the optimizes norbup;s binding to ORs. norbup is a ful agonist and feels better than a nasty antagonistic feeling at 32mgs.

i personally take .5-2mg a day, and i still have a regular sex drive and a clear head after taking buprenorphine. it also has a very normal feel to it at around 1mg, but does have the added benefit of giving you energy like in the morning to wake up, and a nice anti-depressant feel aswell. :)

I agree Cster. On small doses of bupe, my body functions pretty normally.. from sleep to sex drive, etc.

I'm getting off completely soon anyway but it's nice to have the benefits with out all the bull shit. This is why I encourage people to drop to as low a dose as possible.. of course it's important to not go too low before you are ready to deal with cravings or are very serious about being sober. It has to be the right time.
