Thing is if she's intoxicated when she signs the contract then the latter is automatically invalidated.
 
this whole concept is ridiculous, unworkable, a huge turn off and smacks of amateur notions of having paper aids tests to prove your safety before sex.

i remember when i was 13-14 and going out to hook up during my manic phases i would think that i would ask someone if they had any diseases. turns out you dont do that in real life for two reasons

-its a turn off and very awkward

-people lie anyway

Ah, safe words straight from the BDSM handbook. Course, for normal people a simple 'No' would surely suffice? I guess being bound by a legal agreement really adds a whole lot of weight though for someone who'd ordinarily ignore the no and just go with full on rape regardless. Naughty, naughty rapists. 8)

I'm not fucking anyone who I would need to consider signing a sexual consent form because they're gonna say I raped them.

Most certainly. The legal repercussions of fornication are enormous and can be quite expensive or damaging to the mind/body. This sort of agreement should protect the interests of all parties(M/M, M/F, MMF, FFM!, MMMFAliens, ECT) in sexual intercourse.

Our society is irresponsible for not establishing a proper agreement system for sexual conduct.
 
Usually society sees fit to leave it up to consenting adults to work out between them, boundaries, permissions and such like. There's rarely room for confusion here. Where such confusion exists it's most often down to disinhibition by alcohol. So drunk you're not fully in control of your mind and body and capable of full consent or withdrawal of consent would also make agreements like this immediately null and void. They'd have no weight if the signatories were compromised rationally.

I get your point and it's possible that maybe in some rare cases this form would actually be useful but I mean...for one no one would bother signing it (I mean how annoying would it be to have to fill it out each time), and also I feel if you're sleeping with someone you should at least be careful who it is and be sure they're not gonna accuse you of raping them..if you can't have that sort of judgment you should be having sex with them at all.
