Um... this is not only a consent form, but it stipulates what sexual acts you will perform.

It's not even close to enforceable, kaywholed. One of the elements of a contract is mutuality. This utterly lacks mutuality. And there are other problems too.

You're better off having sex with someone you trust and even then there are no guarantees.
 
how does it lack reciprocal agreement? both parties agree we didn't rape each other.

my biggest concern is that it does not address pregnancy.

I always bring my briefcase to the club to cover all bases if I'm going to engage in sexual activity.
 
Not necessarily a bad idea in principal. Perfect for drunk one night stands so you don't get "Tysoned" by a sex partner who regrets what they did when they sober up.

That said it is also pretty silly for 99% of life situations but perfect for drunk one nighters.
