replacing MMT with DHC

Hi guys,

I know someone with a bunch of old 60mg dihydrocodeine tabs, and ive read before that in some countries they use DHC as a maintenance drug instead of methadone.

My questions is, can you go straight from methadone to DHC? the dose is 130mg of methadone and theres plenty of these 60mg DHC tabs to last for quite a while even at a dose of 500mg a day if need be. where would one start? would you take the DHC in one go like the methadone or divide it thru the day? is there any conversion charts someone could point me to?

DHC is considerably weaker. You will have to take very large doses of DHC indeed to come anywhere close to MMT's efficacy as a maintenance drug. 500mg is too little that's for sure.
 
you would have to take a huge amount of DHC to =130mg methadone (ive read along the lines of 200mg codeine=20mg methadone.. then DHC is 1.5 times stronger than codeine) so according to that you would need to dose around 900mg DHC, also take into consideration that DHC lasts nowhere near as long as methadone, you would have to redose 4 times a day probably...

I've also heard of people using DCh instead of methadone (in switzerland maybe), but i think that its from smaller habits than 130mg methadone
 
According to the first post here 60mg of DHC is equal to 5mg methadone and DHC is short acting. This means you would need 1560mg of DHC per day to be equivalent to 130mg methadone. I would suggest given that it is shorter acting you would want to take multiple doses of DHC a day compared to the one dose of methadone. How many doses of DHC would be best I could not say.

I don't know how accurate those conversion rates are but that is the 3rd equivalency chart I looked at and the first one to list DHC. It would seem to me it would be a lot easier for you to stay on MMT.

you're gonna need at least the 500mg from a dose of 130 methadone. i'm on 300mg DHC dailly, after being on 40mg of methadone dailly...so either it's gonna be a big step down. or my doc gave me a step up :D
