Relapsed on H after car accident. Still taking Subutex but it won't stick. Need help.

I recently got in a car accident and got whiplash and back problems(pain) plus massive headaches. . It took a long time to get insurance to set up massage therapy and since I am on 32mg of Subutex there is nothing a doctor could or would prescribe for pain other then ibuprofen. So I relapsed on H. I had one vein that worked for a while and was able to keep the pain under control and get a good hit. And for the most part I kept up with my Subutex. If you hit the vein you will still get high, or IM works too. Anyway, it's been two weeks and even though I've been taking Subutex (maybe missing 3-4 days in the 2 week period) I've gotten to the point where I cannot get back on to my Sub without going through withdrawals. To make it worse I have to IM now because I my veins are shot from years ago and never had good veins. And my shoulder muscles are aching from injecting 2-4 times a day. Today I almost made it. I got to 5pm. I took the sub slowly throughout the day but at 4pm I was feeling so terrible that I gave up. It shouldn't be that hard to get stablize on Subutex. But it's not going well. How can I get back on Subutex again?

Has anyone experienced anything like this?

Any help on this would be very appreciated.

So I think your options here are to either deal with some degree of suffering while transitioning back to Subutex or else go on methadone (personally I would suggest really trying hard to get back on Sub before considering methadone, however). To get back on Subutex you should ideally not use any heroin for around 24 hrs or until you are feeling full withdrawals. You might actually have to wait a bit longer since you're doing IM injections. If you're going to try taking the Sub sooner than that then the only thing I know to do was like you said take a little bit at a time every couple hours to try to reduce the chances of precipitated withdrawal. How long had you waited since your last heroin dose and what dose of Sub did you take?

If you are going to keep using heroin in the meantime perhaps consider "plugging" - rectal administration - as opposed to IM injections.

I'm going to move this over to Other Drugs since they are the experts on switching from heroin to Subutex/Suboxone, so you might get more replies there.
