Question About Liquid Dosing

I know if I were to dissolve 4mg of hydromorphone in 4ml of water, there would be 1mg/ml
but what happens when i dissolve a 4mg pill of hydromorphone, which also contains 156mg of binders and fillers, into, say, 4ml of water? Would there still be a 1mg/ml ratio of hydromorphone? Or would the binders and fillers screw that up?

Some of the filler/binders may be insoluble - they could form a suspension for a short while but settle at the bottom afterwards.

If you are IVing the solution it is important that you use a micron filter to prevent the fillers/binders from entering your bloodstream and lodging in your lungs.
