MY FRIEND HAS OVERDOSED ON HEROIN MY BUDDY HAS A NALOXONE VIAL BUT WE DON'T KNOW HOW TO IV PROPERLY

WE'VE TRIED JUST SQUIRTING ONE OF THE FOUR IN THE PACK INTO HIS MOUTH BUT NOTHING HAS WORKED

THE AMBULANCE IS ON THE WAY BUT WE PLEASE PLEASE really NEED HELP ASAP PLEASE

Don't need to IV it. Just IM it (inject it into a muscle). That is how it's meant to be injected in your situation generally.

That being said, we don't give quite this kind of emergency medical advice here. Sadly, but it would be unrealistic to expect us to be able to do so.

Edit: I would have hoped by now the emergency medical staff got to your friend by now (15min after you posted). The time that has elapsed between when you posted and my response kind of help to show why we don't give such emergency medical advice. Best of luck and my prays are with your friend.

In an emergency situation, IM is the best route. There are nasal-specific formulations.

In some situations, a single 1ml vial will not be enough to revive someone. A single opioid overdose typically requires multiple ampules to keep the person alive, as the displacement is temporary (the duration of the naloxone is much shorter than that of the typical opioid of abuse). For heroin, you would need several naloxone dosages.

Call the ambulance again if they haven't arrived yet.

In the future, please educate yourself and your friends on emergency use of naloxone. The needle exchange in my area has a class which I've taken.
