Hi, I came to this site from searching around in google.
I have cronic pain, with a really high tolerance. Everything I take, is not working.
So far I taken 25 mcg to 50 mcg of fentanyl but the problem is with that, 50 mcg helps just a little but the side effect I get is sleep paralysis right after I start dreaming lucidly. Before this I been taking hydro Morphine at the hospital iv. To me this help the best, cause I always got it directly to the blood stream or same way while doing hemo dialysis. Now that I am home, they give me hydro morphine pills. Its been about 2 months maybe 3 right now that I been on it, its not working. What other ways can I take the pills? Because the cronic pain I don't really want to snort the pills cause I don't think it'll help with the pain long term. I was also wondering if you can smoke the pills? Put it in foil and burn it.
I also do hemo dialysis three times a day, so i think thats why most of the pain killers aren't working.
My choice of drugs are limited because I have no kidney, and doing hemo dialysis to live.
The next drug I can take is methadone and I'm not sure if I should take it.
Please give me your opinion of it.

The melting point of heroin is around 250 degrees, and apparently the melting point of Hydromorphone is 305-315, so it would be easier to vaporize off foil.

Also do you know if hemo dialysis clean fentanyl? Since Hemo dialysis is pretty much cleaning your blood, can fentanyl be cleaned out of it.
 
You have five basic choices for taking pills. Oral, intranasal, intravenous, rectal and smoking.

From my experience, most people that smoke their stuff do it wrong. They burn away a ton of the drug and waste the majority of it.
 
^i dont smoke pills, but i "smoked" my dope.....

i never lost any smoke, you get pro.....i had several glass hooters....precut foil, and a cut straw to grab the dope.... just do small enough hoots, and breath properly.....

i think loosing stuff is a big misconception.....so many people here smoke drugs...
 
I wouldn't suggest smoking pills solely because even if the melting point of the drug is high enough to allow for vaporization, the fillers and binders may not and might burn up and cause damage to your lungs that if you had taken the pills via another ROA, would have been avoided.

My suggestion to you, is if your current pain meds aren't sufficient in managing your pain, bring it up with your Dr. and make sure he/she knows that it's not working properly. Give them the chance to get you the right meds at the right doses before you start "abusing" them and risk the Dr. finding out and cutting you off of them.

Either way I would avoid smoking the pills because that would definitely cause harm to your lungs that would be avoided by using another ROA. You should look into creating a solution that you can put in a nasal spray or if you're ok with needles, look into micron filtering for IV use. Both of these things have been discussed in other threads, so search for them and you should find this info somewhere.
 
you have a serious medical condition - talk to your doctor and let them know you are not receiving adequate pain relief.
For someone with no liver, I would highly caution against trying any other ROA as it will jeapordize your health as well as chances for long-term PM when your doctor finds out you've been fucking around.

What about snorting hydro morphine?
 
so far I have talked to my doctor but the problem is having end stage renal failure, I have limited options for any medication

There is no such thing as hydro morphine. If you mean hydromorphone then your best bet is to plug it but you could snort it. Don't even think about smoking it and by the looks of your post don't consider injecting anything. Think about your health and just talk to your doctor. Get on some oral oxymorphone (Opana) or high dose Oxycontin and just swallow it.
 
The biggest concern with opioids and dialysis is not removing enough of the opioid, resulting in overdose.

Fentanyl is actually fairly optimal for individuals undergoing dialysis. It is not very dialyzable and also it's metabolites are inactive so it is a fairly safe option. Methadone also has these characteristics.

It is likely that you simply have developed tolerance and need a higher dose to achieve analgesia.
 
Do you mean hydromorphone? What about plugging? VERY effective method.

You, however, seem to have a very serious medical problem; and I'm sure telling your doc would be the best option; even if you exaggerate the pain a bit to try and twist his/her arm.

By the way, what's wrong with methadone? If it'll work I say go for it!
 
Like others have said, if you're having legitimate pain despite being on meds, you need to talk to your doctor, not just ask people on the internet or try a different route of administration. Especially with your kidney problems, you need to consult a doctor.

I wanted to point out, though, that it's not the melting point that matters for smoking a drug. It's the boiling point. In addition to just looking at the boiling point, you need to know at what temperature your drug is going to start to break down, because the boiling point could easily be above or really close to the decomposition point.

What makes you think that smoking your pills will be any better for your pain long term than snorting or taking them as prescribed? It probably won't...

But yeah if you're going to try anything on your own, the methadone might be the safest choice. Unlike most opioids methadone is metabolized in the liver, so it shouldn't interfere with your kidney issues. Don't take my word for that though. I'm not sure, maybe some of the metabolites are further metabolized in the kidneys.

plugging kinda seems complicated from all the post i read on this site. I thought it was just putting pills in your butt and thats it. can someone explain
 
There's a thread, but basically:

Get an oral syringe from a pharmacy; they'll know exactly what youre taliking about.

Dissolve the appropriate dose in water (only as much as the syringe will hold; most are like 5ml)
Suck up said solution.

Lube up syringe.

Almost there...

Stick it up your arse; about an inch and a half.

Slowly push down the plunger.

You shound be lying on your back with your legs in the air to avoid spillage.

Undignified I know, but a very effective ROA: I love it (the buzz not the ritual).

Look into ultra low dose naltrexone and an NMDA Antagonist (DXM, magnesium, some medications are NDMA antagonists such as levorphanol and methadone though I don't personally recommend methadone for pain from my own experience) to help manage or even somewhat reverse your tolerance.

As for best opiate? I think oxymorphone is excellent. I'm not a huge fan of fentanyl. Hydromorphone and oxycodone are decent but sometimes I think that morphine is better, except for the excessive intensity of dreaming which makes me feel like I never get any real rest. Can't be good for my brain!
 
I would not smoke them. It seems to me that the best ROA for hydromorphone outside of IV is plugging, and some prefer plugging to IV. I was not a fan of hydromorphone when I was on it for a while, it just didnt work that great for me for whatever reason. Feelings seem mixed on hydromorphone pills, some love them and some think they suck. You could IV them, but the problem with that is after an hour a lot of the effects are gone and youre left wanting to shoot up again very shortly. If youre getting instant release pills I would try to get switched to oxycodone which works very well snorted or swallowed depending on what you prefer.
