Opiate Dose Coverter Charts with Diamorphine (Heroin)

I thought I'd post this since everyone's always asking for a converter that includes Heroin.

There's actually 2 conversion charts. One for pharmaceutical opiates, including pharm' diamorphine (Heroin) and a chart for converting illicit UK diamorphine into methadone.

If you wanna know how to convert your H dose into other opiates, convert it to the methadone dose and then convert the methadone dose to the opiate your going to be using!

Its pretty cool but not accurate at all. I dunno where you live but 2 grams heroin IV is not equal to 50-130mgs of methadone. That just sounds crazy.

the second link is very interesting reading makes me wish i lived in england,,

and i think 130mg of methadone would cover a 2gram heroin habit deoending on how potent the heroin is of coarse

no way in hell morphine PO 10mg = methadone PO 10mg.

morphine PO 10mg = 5mg oxycodone PO

Oxycodone PO 10mg ~ 7.5mg methadone PO

So its more like 10mg morphine PO ~ 4mg methadone po

those figures depend on if you aree takeing methadone on a chronic or acute basis and also you need to take into account cross tolerance no conversion chart can be completely accurate because everybody reacts differently

There's some really good info in there. I do think the there is some weird morph info in the table though, (not what was mentioned above). In their conversion chart, they are saying that 10 mg iv = 10 mg po = 10 mg pr (then they list all 3 ROA's as requiring the same methadone dose). Of course their street are going to look wrong to people the way purity varies!

I just started methadone tretment and the doctor told me it is 5 times more potent than morphine but last 4-6 times longer, and that was the methadone doc, so I am pretty sure he knows what he is talking about.

no way methadone is 5 times as potent as morphine but it does last more than 4-6 times as long it can be as last as long as 10 times longer or even more the two drugs really can not be compared

First time I've seen Heroin equivilancies. Useful as a general guideline. I sort of try and convert to Oxy (even though the high is a little different it is the easiest for me to reference to).

I think the H around here 1/8g=80mg Oxy on average I'd say. Not sure what that would convert to in Methadone but it is one way to describe dosage.


That would be a hard thing for me to calculate actually because my usage varies from day to day (as I'm sure there's lots more out there) for a starting dose. I have consumed 50mg orally before (many times actually) but I think I could 'get by' with less. However, if one was to engage with a 'treatment plan' it would make sense to make sure you were comfortable. That is one thing that sort of keeps me out of it, the ease at which I could edge the dose up and up (at least for a while) .

sounds about right as 10mg of oral methadone is probally about 50mg oral morphine for whoever was saying methadone is not 5 times as potent as morphine....true this is inflated because of oral dosing, but still even a IV dose of morphine is weak compared to methadone.

Guy from Ohio I work with had a half-gram a day habit of the good tanny rocks. He topped out with just 80mg and that completely killed all withdrawl, of coarse he also had 2mg ativan every 6hrs; clonodine patches; and drank beer.
