My doctors finally switched my breakthrough med to Opana IR 5mg today. This was changed from a few months of measly 5mg Roxicodone. I'm prescribed Opana ER 20mg three times a day, so you'd think they would give me something better than the weakest oxycodone possible, a dose thats given to people who get their wisdom teeth out while im suffering with severe nerve damage in my entire arm. I mean, the Roxi 5's weren't all that bad. I got 180 of them each month so without APAP I could just eat bunch of them and have a really nice oxy buzz and great pain relief.
Anyway, I've now been given such a beautiful thing. A 5mg pill of oxymorphone that is not trapped in that damn TimeRx bullshit. So obviously I'm really excited to experience all that oxymorphone has to offer when IVed. Its really nice I get 120 of them each month. Unfortunately, the pharmacy at my pain center didn't have them stocked so i have to wait until monday.
So until then I would really like to get an idea about dosing with IV oxymorphone. I am fully aware that this is extremely powerful stuff IVed so any help would be appreciated. I would like to be safe so I am most likely going to start at a small dose and work my way up because I have plenty of them.
However, I would still like to get a general idea of what dose seems appropriate for me. I'm assuming that oxymorphone has the same strength difference to hydromorphone as oxycodone does to hydrocodone (i.e. oxymorphone is 1.5x stronger than hydromorphone).
So with that in mind, about 12mg of dilaudid is a good strong rush for me. I can go higher but 12mg is usually what I shoot. So would that mean that I should be fine with an 8mg shot of oxymorphone? Or should I go a little lower just because of how powerful it is? I'm thinking 5mg?
