Next step up from tramadol/tridural is?

I was on Hydromorph Contin for a few years, dont require something quite that strong anymore, so went back down too tridural, and take 600mg per day. but need something stronger then tridural and tramacet

my Doctor tells me there is nothing in between of these two? Is there anything thats a good amount stronger then tridural but weaker then hydromorph contin 9mg
 
Is propoxaphene still available in Canada? You could always ask for hydro-oxycodone, at lower doses. Hydromorphone is quite strong, you should have a good therapeutic window open for yourself.
 
the toxicity issue with propoxyphene was a major reason for it being taken off most markets.

sounds like you have a bit of a quack if they can't find you anything half way beetween tramadol and hydromorphone. as entheo said, you have a huge open window there in general terms of pain management.
 
thanks for the replies guys, I was certain there must be something inbetween the two. Are those meds mabey more addictive then the long lasting hydromorph contin? He was quite worried about me getting addicted, and I had too fight with him too get the prescription in the first place because of that.
 
there's always going to be dependence/addiction associated with long term opioid use - there's no way to avoid it unfortunately.

i'd suggest doing some research on morphine sulphate extended release form as well as oxycodone (they're really the next step up if you don't have hydrocodone as a middle ground) and make an informed choice of the pro's and con's of each and present your case to your doctor. that's the way to go about it.

we can sit here and suggest everything under the sun but it comes down to the doctor/patient relationship and his final decision, though, he is working for you remember so stand up for your rights of fair treatment solution.
 
Do you need relief from general chronic pain, or just spurts of breakthrough pain? Meperidine, lyrica, gabapentin, and even methadone could be helpful to you. What is causing the pain in question?

I,ve had constant(chronic) low back pain since 13years old.. The pain is there every day all day. some days worse then others. I,ve been too multiple doctors and top of the line spinal specialists for my area. had MRI's and bone scans done. found there is one buldging disc. I have an extra vertabre in my lumbar. there is joint degeneration. the vertabre themself have multiple spurs growing from them in the middle back. also have some type of Non reactive arthritis, which they say is most likely osteoarthritis. and all at the age of 25.

and they say there should be no reason for back pain... but I kept complaing untill they gave me something stronger... I then went too a light duty job, which is when they moved back down too tridural, it was doing its job at first, but is slowly fading. and im taking much more then the prescribed amount....and I also tried lyrics and gabapentin without success.

So I need some type of long lasting pain med....which is also good for the odd recreational use.. the hydromorph was good for both of those,, but he thinks its much too strong for me at the moment
