First off.. I am hoping this is in the right section to post. I am using my phone and full page won't load..

Anywho!
So let me make this as much a hallmark version as possible and see if anyone can give some advice ;)
I have been disabled for a lil over 5 yrs, but started my back surgies 10 yrs ago. I have been on steady medications since 2009, dr (pain mng) have prescribed me ALMOST everything they possibly can think short of phet-patch due to adhesive allergy.
I currently take 15 mg oxy codone 4xday, but I am finding I run out mid month. Thankfully no withdrawl symptoms, something that has to do with my liver, long story. Due to my weight, they can't put me on another dose a day or up mg. morph and dones do not help with the pain.
When my prx comes in, I know I take more the. Prescribed right off the bat to get ahead of the pain, which I turn gives me the groggy/nod feel. At night, it's ok, but trying to function during the day.. Not so much
A friend recommend I try snort to get the pain edge of with a BAM. Well it worked, but because I prefer that right now feel versus having to wait.. I'm wondering what you would suggest so I am not relying on snorting?
I don't do it all the time, but first time I get the prx I do.. Then to keep ahead of pain I have to pop constantly, so I go they meds faster.
I know part of the reason I like the bam from snorting is the head change/more instant high, without the nod and I know that with me popping and going thru my pills faster, part is looking for that feeling without having to do the action of snorting.
Dr have said I will prolly be on some sort of pain reliever the rest of my life due to my diagnosis combos so want to make sure I last physically, as best I can.
I have my green card and smoke at night.. But holy sleepy time for this lady after smoking!!

I also am a cpp, and like urself only respond to oxy. Where in the wrld r u ?? I've never heard of pain mgt according to weight! R u extremely petite?

I'm in oz and on an absurdly high dose of oxycontin/oxycodone. My weight has never, ever come into the equation, (with the exception of ketamine infusions), I wld be questioning what kind of pain mgt ur under...

For the record I'm 5'8 and 60kg, and female.
 
I'm in the states... 5'7" and about 125 lbs... Not sure conversion.

Here, where I live, they are only allow to prescribe 'acute pain' meds (Vic, perc, oxy codone) 4 times a day, unless between injury, pain and weight, the can fudge it to 5.
They only allow the 4 a day to curb ppl from abusing, selling and/or ODing
(And because of idiots like that, screws ppl like me who need it!! Talk about the NASTY NASTY looks I get when I park in the disabled parking spots, and mind u, I have a permit to park there. All because of the way I look and my age)

Oxycotin, morphine and methadone do nothing for the time release pain. It was giving me side effects and that was about it.
My dr call me a 1-3%.. Meaning that my body will do opposite of what a drug is supposed to help with.
Gabapentin made my nerves more sensitive and swell.. They put me on another nerve med that lasted less then a week, have me a seizure. Come to find out, it was initially made To STOP seizures.. The list goes on

Every time a new drug or topical ointment comes out, I'm a guinea pig cuz they know my body can handle it.

The dr thinks that the only 'time release/long term' drug that might be able to help are patches, but cuz of an adhesive allergy can't use them. So I am stuck with 4-6 hr doses that in all reality, last 45 min to an hr if I'm lucky!!

Roo, sorry I didn't see this answer earlier!!

That completely sux! Did that long acting hydrocodone get past the FDA over there? Aren't there two types of fentanyl patches slso? Maybe ur not allergic to both, or maybe u cud take an antihistamine?

How about hydromorph or oxymorphone? Did u try crushing the oxycontin to make it ir? Maybe I shouldn't be teaching u tricks,- but hey, when in need..

Oh trust me, I know and have tried almost all the tricks...

Never even heard of the hydro morph or oxy morph till today when I found this site/forum. Still not 100% sure what they are. What I do know, my dr did a rest on my liver and it processes meds of any kind faster then normal. So pretty much it kicks the narcotic out at the same time it kicks the time release out.. That's why longer last meds don't work on me. I just swear cuz my liver is ridding me of the nasty /non plant additive.
I have ointment for my adhesive allergy, and only know of 1 type of patch. I can't even wear bandaids for more then and hour & 1/2, other wise my skin starts to puff, turn red etc..

I didn't know they had a time release oxy codone, only know of the regular for break thru pain. I've tried tramadol I hopes it would help my nerve condition, but nope. Right now, the only thing to take the edge off is by snorting 2 of my 15 mg and having to take 1-2 ever 2 hours after by nose or every hour by mouth to keep ahead of the pain. Which makes me run out in less then 2 weeks.

They tried a nerve block on my siatic nerve, but didn't realize my siatic and sympathetic nerve had jumped appt due to my sacral joint dysfunction. So the block went into my sympathetic, which I. Turn gave my crps. Dr said he should have listened to me when I said something wasn't right, so they have to see my for the remainder of my life.
Plus my last 2 back surgeries were of no charge to me since a) I was the first in my state to ever have these implants b) my body rejected both c) surgeon 'guarantee This will fix your pain and mobility issue'... But ended up making me disabled instead.

I think the worst part, is when my daughter understands my pain and asks if I have back pills cuz she wants to play..

I'm sure ur a great mum!! Sweet child of mine understands when I say "sorry honey, but mummy's backs sore today" I have never mentioned the extent of my pain or it's widespread nature. She's just like "oh, ok". Age appropriate.

Now, duh yo ur dr for not doing nerve blocks under fluoroscopy!! They need the precise imaging at the time the needle goes in. Have u ever tried SI joint injections?

Shame about ur liver processing drugs quickly, lol I tried "dear doctor, I think I have a liver enzyme that requires me to have higher doses than normal ppl" line, it's a cyp-inhibitor, but he just raised an eyebrow. Rrrreally the lengths I have researched...

Anyway, I'm drinking my exs scotch as he's here asleep on my couch, I was kinda wound up earlier... Better now :D

Less than 36hrs till my scripts due!!

Fun fun!!! And let me kind if explain.. The worst mother of the year award goes to saying is an underlining joke I have with friends and family. Typing doesn't let u hear the joking tone.. I don't feel like that at all!!!

He did the shot with all the correct equipment, but since both nerve look the same size-ish, unless u MRI and follow to wear it connect during, he did everything to protocol.

My dr ordered the liver test.. I didn't even know it was possible! When I was on dones for almost a year, I told him it made my liver hurt about an hr after dosing.. He is a kick ass doc, it's just hard with all my diff problem. I'm like 1 in 3.5 mill to have all these.
What 'fixer' may work for one, aggravates the other... Etc

Speaking on prx, I gotta leave in 5 min to get mine!! WAHOO!!!! We will chat later.. Thank u for all the kind words!!!

So, in the last hr I've gone into a "flare up", this is not an exacerbation of my arthritic pain, but the burning neuropathic pain. I've left msgs with my drs, both of them, and waiting to hear back. Do u ever need to be hospitalised? Have u heard of ketamine infusions over there? I know of patients here that have them for crps.

Ps; oxy has pretty potent oral bioavailability,- is snorting really any different?

To me, personally, snorting gives me a better relief and faster.. Super con about it, doesn't last nearly as long.

And no, can't say I have heard of special-k fusions. I was givin s
Ketamine topical, gel ointment to put on acute pain It helped to full the sharpness of it, but didn't help me like it does to others ;(
 
So, Roo,

Tell me, uv been in pain mgt in the States for five yrs,- and didn't know about long acting oxy, let alone hydromorphone or oxymorphone?

I'm not having a go, but uv posted that ur pain mgt doc is "top notch", and uv tried almost everything avail?!?

Mebbe I've mixed up where u live. But if I'm correct even hydrocodone ER was approved by the FDA iirc.

No offense, but I'm just checking ur informed. U mentioned the "morphones" hurt ur liver, so I'm wondering what they were.

Lastly, we both run out of meds early, but two wks? That's gotta be tough, how do u mng, and is this the plan for the foreseeable future?

Yes he is one of the top notch. I guess I am a lil confused too.. Cuz from my knowledge, morph, dones, oxycotin are all time/longer release that is 'supposed' to last/stay in ur system 12+ hrs. My oxy's, when I looked it up, said only 4-6 stays in ur system. And I'm a lil ignorant to computer and acronym for ER ect.. If u can let me know what that means, maybe I can clear it up for u ;)

The morph was initially 30 mg slow release (purple) one then they upped me to 60 mg (blueish).

Hydrocodone, like norco, didn't even take any pain away for 20 min, let alone the 4-6 hrs like it said it should.

Again, I just have never heard of the hydro or oxymorphone. If they have a diff name (long or generic) I May be able to tell u if I have heard or had em prescribed before.

I hope this clears up some previous posts.

ER- cud be a lotta things I guess, lol, but that last time I meant Extended Release. eg: long acting formulations.

I just know, as is, I'm not happy with my pain mgt... But there is little left, legal or otherwise to help me right now.

I guess I want All pain patients to have the info that's current & warranted than to research their guts out searching for help like I did.

Anyway, u did not offend me in the least!!! It takes a lot to offend me.. I know ppl have diff ways of talking and presenting things depending on where they r from, so I always keep an open mind.. Unless it is just straight out rude ;)

I've been through a 6 month pain management program (had spinal surgery), was stabilized on meds, then moved onto snorting them … which was temporarily more relieving for pain, but worse in the long run. I also needed more sooner than my script was filled. Long acting would be the best way to go … for your situation ime. Just my 2 cents.

I moved onto heroin so stopped ALL opiates, but that's just me. :)

My problem is that the long acting ones don't seem to work on me.. I've taken morph (300 mg 3x day), dones (10 mg/6-7 a time 3+ a day) oxycotin is like ibuprophen.. It gets frustrating!!! My hubby knows about the pills and how I take em, bless his heart all the things, medically, that he has endured with me.
My body is weirdly awesome, I don't become addicted to anything, except my anti-psycho pills (what I call em) and caffeine.
When I was younger, I was splitting an 8 ball a day of crystal, woke up one day, said I'm bored with it, walked away and had no withdraw.
I had kidney stones that were so large they Thot they were gonna have to so surgical procedure to crush em, they gave me 3 shots dilated (sp?), 2 fetanol, and a anti nausea in the er, I stood up and walked out. The nurse and dr said I should pretty much be comatosed with all that in me, but my body doesn't react like others.. I ended up going in for pre-surgery and I had passed the stones with no meds in my system. They had me sign a release for to provide my medical info for teaching purposes. The release has been done on multiple occasions cuz I'm literally an anomaly.
I'm still waiting for my gold star ;)

I do understand about the long acting ones not having as much of an impact or head high as you mentioned…

People usually develop a tolerance regardless if they have addictive tendencies, and medications do stop working well… and more is tolerated /
Very odd, your situation. It seems like you have a lot of insight into yourself, and have had lots of help.
I hope you find some solutions that work well, or better for your pain.

Great you passed the stones - those can be very painful. :) <3

I am very tuned into my body.. My dr told me when I came in and explained my pain, where, how it felt and drew it out on paper, they pretty much knew it was crps & sacral joint dysfunction, but had to confirm it with imaging & other non evasive tests (praise The Lord!!)
My tattoo artist thinks it's freaky cuz I can tell what part he is working one without looking (like numbers, letters, what shape ect)..

I have always had a super high tolerance to pain killers and pain in general. Apparently I had broke my water and was in labor for 5 days, contractions 3 min apart and I didnt feel one..

At times it's a blessing, others it's a curse.. I'm just glad I found this place, where I can ask quest, bitch and moan, and get support. Due to my age, I get a lot off ppl thinking I'm not disabled or in THAT much pain ect.. But they don't live in my shoes, so they don't have any clue!! It really really pisses me off when someone looks at me and says 'oh I know what u mean about back pain, I pick up a bag of dog food with my back and pulled a muscle'... Takes every ounce of my being to not tell em to shove it!!
Sorry.. Got a lil off subject..

Anywho.. Thank u for ur kind words and advice, I listen, well read, every word ppl write in response to any thread I post personally or comment on, with an open mind and hope one day, someone will have the 'miracle' that will help me in my situation.. Even if it's just for a short time period.
