Its been awhile since i posted, things were going good for a minute. So I have a friend bringing me a Subutex to help with PAIN. My current tolerance is 150 -200 mg morphine daily, and I take 1-2 mg xanax daily. Am I going to be good with this? Any problems. I take meds orally or snort, so which would be better? Half or whole to start with? Leery, but desperate and in a lot of pain!!!

I guess you have an 8mg pill? Taking it orally won't work because Buprenorphine has a very small bioavailability that way. The pills are designed for sublingual administration so I suggest that route although you can snort them.

With your tolerance I would try about 4mg since it is roughly equivalent to your daily morphine dose. Btw how do you take the morphine?

Regarding the Xanax as long as I know Subutex is less dangerous in combination with other CNS-depressants than morphine, so you should be fine.

Take 2mg's and see how you feel. Bupe can take up to 2 hours to fully kick in, so give it time. If you feel you need more, take another 2mg's. I don't imagine you needing more than that for that size habit.

150-200mg of morphine IV, oral or insufflated?

if oral, that's equal to 15-20mg of oxycodone, in which case i'd recommend you take no more than 0.5-1mg of subutex, 0.5mg probably being more than enough.

if IV i'd take 2-4mg of subutex

if insufflated i'd take 0.75-1.5mg of subutex.

That's really bad HR info, in my opinion. Buprenorphine is still a CNS depressant, and mixing in more depressants could prove to be fatal. So telling someone it should be fine, is kind of wreckless...

I'm not trying to start shit or come across as an asshole or anything, man.. I'm just sayin'...

If you look at the Bluelight Opioid Conversion Chart 10mg. morphine =.3 mg buprenorphine. You will have to figure out what your dosage equivalent is for your r.o.a. I would go with 4mg sublingual, 3mg.insufflated, or 1mg. iv if you must or only have a limited supply of subutex. You could titrate up every hour until you find a dose that keeps you from rattling.

Thanks all:) I just snorted half of it (4mg) and that killed enough of my pain to make me sane again. Sucks when you run out of your scripts a few days early and have to scramble to find pain relief some other way. I'm pretty cautious and don't like trying new but have no choice when theirs no tried and true available!

So just as an update, I found that 4 mg taken sublingually will pretty much hold me, but 6mg is where I like to be and what I take if rationing is not an issue. Compared to the morphine I take orally, it is almost equal in pain killing effectiveness, and since I take my meds for pain I never notice any "euphoria" with either one. I do notice that there is one HUGE difference! Morphine is way more sedating to me, makes me lazy and not wanting to do anything but sleep or watch tv. The sub actually gives me more energy, which on the plus side, things get done that normally wouldn't, but on the bad side, I tend to overdo myself and end up really paying for it later. I also noticed that with the morphine, I find my mind constantly calculating when the next time is to take my doses, or if I took it all at once, when I can get away with taking some more safely. With the sub, I take one dose a day and my mind is then free to go other places lol. I don't think about re dosing at all. Unless is towards bedtime and my pain is creeping up too high. But my mind is not occupied with it. Even though Im a CPP, I've been thinking about actually telling a Sub Dr that I'm an addict just to get them for pain relief. I have a physical dependency already and addiction runs in my family, so I'm very cautious with my usage even though I'm Rxed my meds. Anywhoo, thanks for all the advice and comments! I love this site and the people here who do such a great job contributing and moderating.

I have fibromyalgia, so nothing will ever take away 100% of my pain. I've found that morphine works best for controlling most of my pain. My Dr has been really great in working with me to find the perfect balance of meds. For the first time since 2010 when all this misery started, I am not being under-medicated. Right now, I recently got an increase in my meds, now I take 60mg MS Contin ER three times a day, and he replaced the morphine IR 15 mg three times a day with Percocet 5/325 1-2 three times a day, with the option to increase those to the 10/325s if I'm always having to take two at a time anyways, to decrease the APAP intake. Now I am comfortable, my pain hangs around a 1-2 with no activity, and maybe a 3-4 with a lot of activity. Morphine does nothing for the nerve pain, I have to take gabapentin for that. Tolerance does build quickly though, idk if it's like that with all pain meds, but Ive found that to be true with hydrocodone, oxycodone, and morphine.
