Narcan / naloxone

Id love to hear from anyone who's ever had an opiate overdose reversed with narcan. Ive administered it many times (usually 400mcg IV plus maybe another 400 IV or IM.
Anyway how doesit feel? Are the precipitated wds really awful? Some people are FAR from greatful even though youve just saved their life....

The 400mcg IV followed by 400mcg IM trick was taught to me by this much older paramedic who reasoned that if they fuck off as soon as the shot has brought them back then due to the short half life of narcan they could go under again in minutes....at least with the slower release IM shot theyve got a fighting chance to get to hospital of their own accord before they expire.

Also he taught me to give 400 of narcan to any overdose even if they say they havent done opiates cos it cant hurt and folk arent always truthful.

Anyway would love to hear anyone's experiences. <3
 
only secondhand im afraid but from a very good mate...od'd in a public toilet the paramedics were there in minutes (his GF called 999 straight away) hit him up with narcan and was really fucked off as as far as he was concerned he was having a lovely nod..anyway he refused to go to hospital and was cooking up another hit as the ambulance drove off in to the distance (he's dead now btw)
 
Youd be amazed how often that happens mate. One minute theyre in nod land and then next theres a couple of blokes in green saying "you alright mate" while the guy is in wicked precipitated withdrawals. The fact that if youd left them theyd be brown bread escapes then. Ive been sworn at and even attacked (although as maybe you can imagine Im NO-ONES punchbag....couldnt give a fuck who they are...)

But yeah hence the second IM narcan just incase they do something stupid like run away and cook up again. Not guaranteed to save them but better than nowt.

Although we do have the power to force people to come to hospital without their consent if we deem them not to have mental capacity to make their own decisions. Everyone has the legal right to a bad decisions but if its deemed you dont have the mental capacity (either permanantly or just temporary due to the state you are in, then you can be forced to come with us (requires calling the old bill for our protection but the mental capacitt act is a purely clinical descision and isnt taken lightly.....Stee probably knows a bit about it. From his nursing work as will consumer.
 
My experience is from your side of the gurney-and getting yelled at by the patient because we killed his high and it was SO expensive!8(

More or less exactly this (aside from that last bit cos I've not seen him in years so have no idea - was my best friend for many years though and was at least tangentially involved in all the more extreme tales of times past I tend to tell. I've also known many, many more folk at more of an arm's length who report the same in such conditions.

My only personal approximation is when I was in inpatient detox and took part in a pilot study. Said study mostly involved upping my bupe dose from 2mg up to... memory fails but either 16, 24 or 32mg. Concurrently my naloxone (no, not quite the same but close enough to have significant similarities I suspect) dose was doubled each day. I've been through many a withdrawal but precips are just horrendous in my... somewhat related experience.
 
I was given it after I went over in 2008 but I had been 'clean' for about a year. Of course I was still chipping from time to time and my tolerance was low which I guess is why I fell out but because of my usage at the time I didn't get precipitated w/d's. I had begun to regain consciousness by the time the paramadic had the cannulla sited and the drug ready so I asked him about whether it would give me w/d symptoms considering my usage was so low at the time. He said hed put half the dose through and give it a minute before injecting the rest to make sure i wasn't unduly uncomfortable with the effect. The half dose did prang me back to life somewhat, but apart from feeling super alert there was no discomfort at all so he gave me the rest of the dose. I didn't require a second one but it took 5 hours of inpatient hospital care to allow my breathing to recover and for my O2 sats to reach a level where they felt it safe to discharge me.
 
Yeah i sometimes give half and titrate but if im honest disposing of half used amps is a pain. Dix it take long for your sats to come back? Usually 100% O2 at max flow of 15 litres/min will bring it back in about 2 mins...of course i know nothing of your history bro and that dose of O2 given to someone with COPD will send them into hypoxic drive and kill then (which is why its idiotic to let firemen give full strength O2 (which is still a prescription drug) to just anyone.....i mean a fireman put fires out and rescue people, the old bill (not my favourite people.....tell more lies that the crims IME) arrest people and ambulance administer emergency medicine and take people to hospital.....its horses for courses.....you wouldnt aske a roofer if hed mind digging these foundations on this new house for fuck sake!!!!!

The maximum dose of narcan we canl lagally give IV in a paramedicine situation is 4400mg (11 amps)....i guess if you not back by the you aint coming back!!...

I think we need to get some poor sod a volunteer who is currently opiate dependant to go through (with proper supervision and medical assistance to hand, I cannot overstate how incredibly irresponsible this would be to do on your own) the full cycle of life-threatening hit to precipitated W/Ds and report back with a full description of what it feels like to be slam dunked all the way from the peak of peaks to somewhere in hell or possibly Milton Keynes. Then again, I would think we'd have a hard time finding someone on here willing to put the time and money soley toward intense suffering. Maybe they could get it funded as a research project like that one guy with the bee stings...

Whats wrong with Milton Keynes? I am quite fond of it ;)
