Opioids Naltrexone = sensitive to lower doses of opioids after naltrexone is stopped?

Anyone have feedback on the Naltrexone/Revia package insert note that says "Patients should be aware that they may be more sensitive to lower doses of opioids after naltrexone treatment is discontinued."

I was on 12.5mg for a bit, stopped occasionally, and two days ago took my last 12.5 dose.

Likely will be using Opiates three days after my last dose.

Usually use 4mg IV of my substance of choice (Dilaudid). Any ideas as to a smaller dose or first hand experience?
