my combinations of drugs...

Hi all, I'm new to this site but it looks like I will get some important info from Bluelight.
About me:
I am a 56 year old Type 1 Diabetic since I was 9 years old. I've been on an insulin pump for about 15 of my 46 year stint with diabetes. Having diabetes for as long as I have results in a number of ailments andmy list is: neuropathy, charcot feet, gastro paresis, constant nausea, retinopathy, GERD,
hypo-thyroidism, heart disease, angina, high cholesterol, osteoporosis, dry mouth, and sleep apnea.
I am prescribed the following: Novolog insulin, Prozac 40mg, Fluoxetine 40mg,
Neurontin 300mg, Vitamin E 400IU, Synthroid 175mcg, Dexilant 60mg, Vit. D3 5000, Zantac 300mg,
Lovstatin 40mg, Melatonin 10mg, Losartan 25mg, Plant Charcol, Promethazine 50mg, Biotin Spray PRN,
Minitran Patch 0.4mmg/hr, Nitrotab 0.4mg, ASA 81mg, Flagyl 250mg, Boniva, Zolpidem 10mg, oxygen
when sleeping.

The two drugs medical professionals can't believe my doses are:
Phenergan 50mg four times a day
Neurontin 600mg four times a day

Neither of these high doses affect me adversely. The Phenergan keeps the nausea at bay but doesn't knock me out. The Neurontin doesn't affect me adversely when I take the above dose but I'm having an especially bad experience with neuropathy in my feet right now. I have taken 12-300mg (3,600mg) of Neurontin today. The pain is finally subsiding and I'm just a little bit buzzed.
When does an overdose occur and what are the symptoms?

We aren't doctors here, however, your doses of Neurontin and Phenergan are fairly high. I would not be too concerned with the Neurontin, so long as you aren't experiencing balance problems, or problems with judgement. I am not sure taking that amount (or any) of Phenergan daily is a good idea as it is a "dirty" drug, and you could experience some side-effects that aren't pleasant. You say that you tolerate it well, though, so it is not too concerning in the short term, but I would suggest switching to maybe Zofran or a better anti-emetic if you need daily nausea control. DON'T move toward anti-emetics that are neuroleptics like Reglan or Compazine for example if you are going to be taking an anti-emetic daily. I don't know a ton about some of the other drugs listed, so I can't comment much on them. Put I gave you my thoughts on the Neurontin and Phenergan. Also, Prozac is just a brand name for fluoxetine.

Those doses are fairly high, it's unusual although not unheard of to be prescribed more than 75mg promethazine (Phenergan) on a regular basis but your gabapentin (Neurontin) dose is not out of the normal range.. If they work for you and your doctors are willing to prescribe them then I don't see a problem. This is a medical issue though so something you should talk through with your doctor.

3600mg is the usual max daily dose of gabapentin. If you are taking more than your prescribed dose though you really need to discuss it with your doctor. If the gabapentin isn't cutting it then there are other drugs they may recommend such as pregabalin. Good luck!

Neurontin can cause nausea vomiting and slow down your digestion track. It can cause acute renal failure. Since your already diabetic I would watch for how much you pee. If you are not peeing a lot, the normal is 60ml a hr, I would look into that. Phenergan interacts with Prozac. Prozac causes increased blood levels of promethazine which can cause heart and urinary problems. They both mess with your blood's RBC and WBC decreasing both.

Gabapentin shouldn't cause renal problems at a stable dose unless you become dehydrated (which you should be careful about anyway being diabetic) but that's a good point and shows why it is important not to increase doses of medications without consulting a doctor - if you do have diabetic nephropathy then the maximum safe daily limit might be less than 3600mgs depending on your renal function.

If you are not experiencing side effects from the promethazine and your doctor is happy to prescribe that dose then it then that's okay but let them know if you do experience any adverse effects. As promethazine has antipsychotic properties one of the things you might notice is akathisia (restlessness) or other movement disorders - let them know if you have any concerns. I'm not aware of heart/urinary problems being common side effects of promethazine, although cardiac conduction disorders would be a possibility I imagine.. speak to your doctor if you have any concerns however
