My brother is on a alcohol + adderal bender -- how can I advise him?

My brother ("B") relies on me for informed advice and guidance, so I'm doing his homework for him.

B is in his late 20s/early 30s, about 200lbs give or take, and has a history of drug experimentation with no major health problems.

B was 100% sober in January. B has been on stimulants to treat his ADD since age 13. Most of his life was was ritilin, but 6 months ago, switched to adderal.

On Tuesday night, from 7pm to 3am, B took 40mg of adderal and drank 400ml of whiskey (spread out over the evening). B felt fantastic and joyfull. B made sure to supplement with vitamins, hydrate, and eat food. B never felt drunk or like he was loosing control.

In the morning, B felt crummy... a kind of "low blood pressure" feeling. Slightly nauseous, but not impaired. Work was slow and unfocused. B wanted to lie down and take it easy, but felt normal in the evenign.

On Wednesday night, from 7pm to 3am, B took 60mg of adderal and drank 400ml of whiskey (spread out over the evening) B felt fantastic and joyfull. B forgot to eat food + supplement.

In the morning, B felt awful with headache + non painful pressure in chest. By evening, B felt ok.

on Thursday night, B repeated and took 60mg of adderall and drank 400ml of whiskey throughout evening. B stayed up later, but still got 6 hours of sleep before waking up.

In the morning, B felt pretty awful, so he ate a big fast food meal. B felt pretty slow throughout the day, but was fine by evening.

on Friday night, B repeated at took 60mg of adderall and drank 400ml of whiskey. B fell asleep at midnight and woke up at 3am "ready to go."

It's now saturday and B has been up since 3am, and is feeling terrific. Alcohol + adderal makes B social, happy, focused, turned on, adventurous, and engaged. B feels non painful pressure in his chest and lower throat, normal heart rate, but heart rate quickly accelerates when B walks up stairs. B has not blacked out or experienced moments where he looses control.

But B also knows everything comes at a cost! B is worried about cardiac arrest. B is worried about seizures. (B has had neither) B has told me that he knows this is really dangerous behavior. B knows that adderall and alcohol is a dangerous combination. B will restore sober normalcy on Sunday, but B is really concerned. B doesn't know if he's mildly harming himself or seriously harming himself. B is shocked that he's gone through 2.5 bottles of jameson in 5 days. B wants me to monitor him and know what signs to look for if B is in potentially serious medical trouble. Will B suddenly fall into cardiac arrest, or will he see it coming? Is B in danger of serious trouble?

How should I take care of B?
 
B smarter, we know it's you no need to hide it. Drinking half a fifth of whiskey and 50mg of adderall is obviously terrible for you. Yes you will destroy yourself if you keep this up. at the very least try not to mix them, pick one or the other and stick with that for the night.
 
Drink lots of water but be sure to replenish yourself with an appropriate amount of electrolytes. Both adderall and ethanol (especially) rob the body of hydradtion although the mechanisms for each are different.

Eating food probably doesn't sound to good, but if you can force something down it will help you in the later stages such as the comedown and hangover. Bread and water simultaneously is a good way to choke down necessary calories. Bite off a small bit of bread, sip some water and swallow. Depending on what kind of bread you like, it should take care of some of your bodies needs for electrolytes as well.

Key: Water is your friend, but too much water without salt replenishment is just as bad as dehydration itself. A small amount of a benzodiazepine or like sedative will help feed your GABA receptors without wreaking as much havoc on your body. Also, an OTC sleep aid such as Benadryl (diphenhydramine) or Unisom (doxylamine) might help you to relax a little bit if you don't have access to anything stronger, but don't overdo, as the side effects of the antihistamines might make you feell worse.
 
The way I see it if you are that concerned I would stop taking those drugs in that way. Like said earlier, pick one or the other. Personally IMO alcohol and adderall are both really bad for your health Taken as you stated.

You possibly could have done slight body damage, but I doubt it. I've gone way harder then that (not saying I didn't do damage, but nonetheless) and as long as you take care of yourself during recovery I'm 99.9% sure you'll be fine.

From what I've read, seizures and cardiac arrest are usually accompanied by a preexisting medical conditions. I have read reports of it just suddenly happening without a medical condition , but it was usually due to an extremely high dose (which that your taking is not that high of a dose) or a very very rare freak accident.

Really though man, just try to work on self control. I'm guilty of lacking skills in that department too but if you are really that worried, I wouldn't want you to get stuck in a routine Of combining those two drugs For a long period of time.
