This remarkable case series describes 12 patients with severe MDMA (ecstasy) toxicity brought to San Francisco area hospitals from a single “rave” party at the Cow Palace in May 2010. Two of the 12 patients died, 8 required endotracheal intubation, 6 were hypotensive, and 10 were hyperthermic ( up to 109.4oF), and 5 were dialyzed for acute renal failure, hyperkalemia, or metabolic acidosis. All patients developed rhabdomyolysis during their hospital course.

Hypotension, hyperthermia, and hyperkalemia were associated with worse clinical outcomes. Of the 10 patients who survived, one required bilateral below-knee amputations for compartment syndrome and lower extremity necrosis, 2 had persistent cognitive impairment, and 1 was still on hemodialysis at time of hospital discharged.

Three patients had serum samples tested for drugs; MDMA was the only substance that was identified. Two drug specimens confiscated at the rave party were tested; these contained 82% and 98% MDMA, with one sample containing two or three times the usual MDMA dose. Despite initial speculation that the extensive and severe toxicity stemming from this event must have been due to “tainted” drugs, MDMA was the only substance identified.

So, what, it was just much more pure than they were used to, and they overdosed on MDMA?

But isn't that strange? People regularly use 100% pure MDMA, and don't overdose.

In any case, we can blame the drug war for making careful, safe distribution of MDMA illegal.

What does a genuine MDMA overdose look/feel like? Can anyone link to a trip report (or whatever)?

In any case, we can blame the drug war for making careful, safe distribution of MDMA illegal.

As that blog says: "Poison is in everything; no thing is without poison." Indeed, having to buy shit without a clue as to the dose you've got is not ideal.

People who have known of people on the manufacturing side of things know, especially with pressed pills (as it appears these were) that someone pressing pills is going to want their first wave of pills to be REALLY good since they are identified with a specific stamp/colour etc.

I would bet that the pills were nothing overly special, I have weighed pressed pills before and even the ones on the visibly large side weighed under 350mg.

Even if these pills weighed 400mg each I could not imagine them having more than 200mg of MDMA per pill at BEST! The reason these people got hurt is likely because they were used to garbage pills that they needed to eat 8 of to get good and high. Sadly this time they got the real deal.

I also wouldnt be against the thought of this being caused by impurities from poor synthesis (I could be way off though)

A frightening reminder of how you can take something every weekend for years with no problem and then something like this happens.

Two capsules confiscated by police at the event contained 82% and 98% MDMA, respectively, without other pharmacologically active compounds. Capsule #2 contained 270 mg MDMA, which is more than twice the amount of MDMA usually contained in 1 dose.
It sounds like they got molly in capsules containing multiple doses and they thought that it was like a regular pill. They were probably used to 50-80mg bullshit and took a bunch.

I wonder what the other 18% was, or even the 2% for the 98% MDMA(fucking almost pharm grade)

Capsule 1, total weight, 135mg, contained 110mg MDMA. Capsule #2, total weight 275mg contained 270 mg MDMA, which is more than twice the amount of MDMA usually contained in 1 dose.

Whoa shit, no wonder there were people overdosing. That's a fat cap. If you had some kids chowing down on 2-3 caps they could be ingesting truly fucking ridiculous levels of MDMA... let alone kids who eat them like candy..

The caps had a small (<0.1% ) impurity of MDE, not enough to be pharmacologically relevant though. Impurity is probably salt.

Looks like the serum sampling showed three people being admitted with between 1.3mg/L and 0.4mg/L serum MDMA. I.e. humongous overdoses. (ref range 0.1-0.25 mg/L effective, 0.5-10 mg/L potentially lethal)

So it looks like the moral of the story is: if you or your friend take a massive overdose of MDMA, bodily cooling can save your life if administered fast enough. Most of the damage seems to be from MDMA induced hyperthermia.

And weigh your shit out, so you don't end up dropping massive 0.3g bombs.

Overdose
Due to the difference between the recreational dose and the lethality dose, it is extremely rare for a death to be accredited just to the consumption of MDMA. While a typical recreational dose is roughly 100–150 mg (often being measured by eye and dealt with as fractions of a gram), this dose is often then repeated but remains well below the lethal dose. Consumption of the drug can be self-reinforcing while under the influence, and overdoses can occur. People who are grossly obese, or who have diabetes, high blood pressure or heart conditions have a greater risk of overdose death from any stimulant, and should generally avoid MDMA and similar drugs.[citation needed]

The standard treatment for ecstasy overdose given in hospitals includes a range of drugs such as cyproheptadine or chlorpromazine[30] but these are often of limited efficacy. MDMA overdose mainly results in hyperthermia and hyponatremia, which leads onto convulsions from the hyponatremia and rhabdomyolysis (toxic muscle breakdown) from the hyperthermia. These complications can be treated; benzodiazepines such as diazepam or lorazepam are used to control convulsions and dantrolene blocks rhabdomyolysis.[31]

It has been argued that "the seriousness of the effects can be dependent on environmental factors other than the drug concentration", as blood concentrations of the drug spanned a large range in cases of death in MDMA users. This not-with-standing, "most of the cases of serious toxicity or fatality have involved blood levels... up to 40 times higher than the usual recreational range." [32]

Quoted from Dr. Julie Holland: "Not only are MDMA related cases a small percentage of all drug-related emergency room visits, but a large percentage of MDMA cases are not life-threatening. In a recent study conducted by the physicians in the Emergency Department of Bellevue, (Rella, Int J Med Toxicol 2000; 3(5): 28) regional hospital ecstasy cases phoned into the New York City poison control center were analyzed. There were 191 cases reported during the years 1993 to 1999 inclusive. This is a rate of fewer than thirty cases per year. 139 cases (73%) were mild and experienced minor or no toxicity. The most commonly reported symptoms were increased heart rate (22%), agitation (19%), and nausea and vomiting (12%). In these seven years, only one ecstasy-related death was reported, which was due to hyperthermia, or overheating. Ecstasy is simply not the "killer drug" the media would like us to believe." [33]

way to many fake pills cut with R/C's that mimic the Mdma high...

last good pills around America in NYC area have been G Ladies in white color in around 2004-05

150mg-175mg was like the dosage of those G-ladies back in the day.....

Why is it hard to accept that the same retards that would take 8 bunk pills and claim to be "rolling hard" would be the same retards that would take 8 awesome pills and "overdose hard?"

It sucks but it very well could have been good ole fashion MDMA. You know people can overdose and die. Fair warning :)

Also I always here about these famous "g ladies" all the ones I had sucked. Not saying the ones a decade back werent good but eh crystal is the way to go, properly weighted and rammed in the ass for maximum effectiveness!

I've seen tox reports that suggest that anyone who exceeds about 1.5 grams of MDMA administered is going to have a pretty fucking rough time. At the very least you'll feel like hell, at the very worst you cook to death.

One ER report that sticks out in my mind is someone who dosed ~100mg MDMA, decided that it was bunk, and injected the remaining ~2 grams IV. I bet you he would have had a much nastier outcome had the EMS guys not been involved.

If you look at the states in which these people were admitted, it's amazing so few of them died at all. MDMA is definitely reasonably safe in overdose, but when you start taking 20x the active dose of the drug you have to expect problems.

Even 20x the typical dose of caffeine (assume 100mg x 20 = 2g) would make most people sick to their stomach. A 20x overdose of alcohol would leave you pretty haggard if it didn't outright kill you.

Pretty much the only drugs I can think of that are more benign are things like LSD and cannabis, but even then a 20x overdose can utterly fuck some people's heads.

People make stupid choices. MDMA can kill you if you don't dose it right. No drug is ever perfectly safe. It's not that hard of a concept to grasp.

Yeah good point I guess. Seem pretty weird we havent had any OD deaths here in Aus (that i have ever heard of anyway) in over 25 or 30 years of people taking mdma, often in very large doses, the only deaths i have heard of was one girl who was 15 and had a really strong pill and died from water intoxication and others who had pma pills. Even tho our population is pretty small compared to other places, we apparently are the biggest drug takers.

So with all the party heads out ever weekend here over the last 30 yrs and hardly any major harm (as far as i can tell directly from mdma) and even people often taking very large amounts of very strong pills, still hardly any, if any deaths. Seems pretty safe to me. More so than alcohol and heroin and gbh which i've read of so many deaths from OD directly from the drug.

But as you say, it can happen, especially at a 20x OD.

Something to consider: Most people who do consume lots of MDMA don't consume it as one bolus dose. Taking 1 gram of MDMA in small aliquots over a period of ~6 hours would be much less harmful than taking it all at once.

It may just be the case that these were people who decided the reward of eating multiple pills at once outweighed the risk.

I guess it must have been. And possibly Capsule #2, total weight 275mg contained 270 mg MDMA, dont know how many of them ate pills or had caps like that, but yeah I get your point.

What does a genuine MDMA overdose look/feel like? Can anyone link to a trip report (or whatever)?

Adverse effects

Also, serious adverse events in MDMA users may be an interaction of the drug with a preexisting medical condition. Risk of adverse event after MDMA consumption is thought to be increased by preexisting cardiovascular problems, such as cardiomyopathy, hypertension, viral myocarditis, and congenital cardiac conduction abnormalities (such as Wolff–Parkinson–White, Romano–Ward, Brugada, and Jervell and Lange–Nielsen Syndromes).[15]

Serious adverse events in MDMA users may also be caused by drugs sold as "ecstasy",[16] but which are not actually MDMA. Dangerous overheating, sometimes fatal, is associated with drugs such as PMA[17] or 4-MTA.[18] To help mitigate risks associated with the consumption of MDMA, certain organizations have created screening test kits to prevent the consumption of more harmful substances such as PMA, Methamphetamine, 2C analogs, BZP and TFMPP.[19]

[edit]Hyponatremia

An important cause of death following MDMA use is hyponatremia, low blood sodium levels as a result of drinking too much water.[20] While it is important to avoid becoming dehydrated, especially when out dancing in a hot environment, there have been a number of users suffering from water intoxication and associated hyponatremia (dilution of the blood that can cause swelling of the brain).[21] Although many cases of this clearly involved individuals drinking large amounts of water, there are cases where there is no evidence of excessive water consumption. Their cases may be caused by MDMA inducing release of the antidiuretic hormone vasopressin by the pituitary gland.[22] The action of vasopressin on the renal tubules leads to the retention of water, resulting in users producing less urine. (This is unrelated to having difficulty passing urine, a phenomenon known colloquially as E-wee).[23] Hyponatremia also affects marathon runners and bodybuilders, who have been known to die from similar causes, as a result of drinking too much water and sweating out too much salt. It affects women more than men.
Hyponatremia is preventable by drinking fluid containing sodium, such as that contained in sports drinks (typically ~20mM NaCl).[24]

[edit]Hyperthermia

The primary acute risks of taking MDMA resemble those of other stimulant amphetamines. The second most important cause of death from MDMA use is hyperthermia, core body temperature rising too high until the major organs shut down at about 42°C. This is comparatively more problematic than blood salt imbalance, harder to treat and to avoid. Ecstasy-related hyperthermia may occur as a symptom of serotonin syndrome, which is where too much serotonin is released into the brain. This can occur with MDMA if too much 5HTP or other serotonergic drugs are consumed together. 50–200 mg of 5HTP is believed by some users to make MDMA work better and last longer, but anecdotally more than 300 mg 5HTP may increase risk of serotonin syndrome[citation needed], which can lead into lethal hyperthermia if it becomes too severe. It has been suggested that hyperthyroidism may also increase risk of ecstasy-related hyperthermia.[25]

Note that this is different from normal hyperthermia. Dance parties are an obvious hyperthermia risk environment, the venue often being hot and crowded, and the attending public dancing whilst on stimulant drugs. Ideally the temperature inside the dance rooms should be maintained in the range 24–27°C; ecstasy affects the body's ability to regulate temperature and it is easy to become either too hot or too cold if the temperature is outside of this range.

Mild hyperthermia and/or dehydration can occur from dancing too long, and users may recover with administration of fluids and rest in a cooler environment. However, if the user expresses concern about how hot they feel, or if their body temperature is still rising even when they have stopped dancing and are in a cooler environment, and their skin is hot and dry to the touch, then they may be developing more dangerous drug-induced hyperthermia, and these cases should be taken to and handled by a medical professional immediately. Treatment is most effective the sooner it is given, as with all adverse drug reactions. Hyperthermia is a particular concern if MDMA use is combined with other substances, such as 5HTP, or if additional stimulants are involved, such as methamphetamine or cocaine. MDMA is also implicated in affecting the mechanism of uncoupling protein (UCP), more specifically UCP3 in mitochondria which can lead to the abnormal thermogenic response.[26]

In animal studies, a combination of prazosin (alpha 1 adrenergic antagonist) and pindolol (5HT1A antagonist/beta blocker) quickly and completely terminates drug-induced hyperthermia.[27] Another drug, the migraine medicine pizotyline has also been shown to be useful in treating MDMA overdose in animals.[28] However, neither of these treatments are approved for use in humans.
MDMA appears to decrease heat loss in the body by causing constriction of blood vessels near the skin. In addition, it can increase heat production by muscles and the brain. These effects may be amplified in people who become dehydrated and are therefore unable to cool by sweating. On top of this, MDMA can mask the body's normal thirst and exhaustion responses, particularly if a user is dancing or is otherwise physically active for long periods of time without hydration. Because of these effects, MDMA can temporarily reduce the body's ability to regulate its core temperature so that high-temperature surroundings (e.g. clubs) combined with physical exertion may lead to hyperpyrexia if precautions are not taken to remain cool. Sustained hyperpyrexia may lead to rhabdomyolysis, which in turn can cause renal failure and death. Depending on the initial cause of rhabdomyolysis, it may be successfully treated with dantrolene if diagnosed early enough, but often the characteristic symptoms may not be apparent until the condition is already severe.

[edit]Treatment

Ecstasy induced hyperthermia may be treated with dantrolene.[29
