MS Contin to Naltrexone advice?..

Hi peoples..

I was looking for people with experience of the transition from opiates to naltrexlone (the full opioid antagonist) used to prevent relapse..

Ive been a heroin addict for 15 years nearly, and iv'e tried every treatment option going over the years.. been to rehab 6 times.. done ibogaine 4 times.. etc etc..

So, my psych has given me a MST (MS contin)((Morphine Suphate in slow release tablets)

Im currently taking 200mg twice a day, the equivalance (using online guides) is anywhere between 50 and 70mg methadone.

Anyway, the only way I ever got clean before, and stayed clean for 2 1/2 years was by tapering methadone.. so Im going to try again, only this time with morphine.

The idea is when I get to like 20mg, to stop and go onto naltrexlone tablets (NHS wont pay for implant)

My psych seems to think I need to wait 5 days inbetween to avoid precipitated withdrawal but from researching it and talking to friends there are plenty of horror stories out there of people swapping over, nibbling a nal tablet after 5/6/7 days and going into full blown horrendous WD!...

Once youve taken that tablet you cant reverse it.. youre going to be sick if theres any opiates in your sytem still.

Now, it's easy enough to say 'wait 5 days' or whatever, but my problem is I can never get that clean in the first place!

Im debating taking a small dose of ibogaine again at the end of the taper to kick off any remaining opiates before the Nal

And if I don't do that, im going to leave it a LOT longer than 5 days..

So my question is, does anyone have personal experience of the transition?
What's your opinions on days clean in-between?

Apparently nibbling a 20th of a Nal tablet is basically no different to taking the whole thing...

Any advice appreciated!

Yea I hear you your doc doesnt really expect you to kick CT for 5 days does he? With the right combination of comfort meds it could be done. Whats your plan to deal with PAWS?
 
this does not feel very basic.

i'm going to send it to OD. you should also search around OD, as i imagine there are threads and mega threads already dealing with your questions. they might call this specific thread self-serving and close it. they might merge it. they might leave it be. dunno.

Hey OP,

First I am going to note that naltrexone is not going to help with anything except preventing you from taking opioids because they won't have any effect. So as you said your real problem lies with getting clean so you can get on it. Once clean if you aren't comfortable waiting 5 days before taking the naltrexone you can wait a week, or two, or even a month, so long as you can resist the urge to use. Naltrexone isn't going to help you get clean only stay there.

What you have to focus on now is how to get off of opioids; this is something we get asked a lot. A morphine taper is a good plan and I wish you luck.

I won't merge this thread in there as this is about how long to wait once clean before taking naltrexone, but you still have a challenging road before you get there.

PAWS is what gets me every time.. Iboga is fantastic for the acute WD period but PAWS always seem to creep back in and I struggled.
