what's the most APAP you'd ingest (ideally none I know) before considering a CWE? The most APAP I've ever ingested was taking 8 Tylenol 3's (2400mg APAP). I regularly take 4 Norco 10/325's and am wondering what other people consume before deciding to do a CWE. I know they say 1000mg per dose is the max and 4000mg per day but there must be some buffer zone safety wise...

I never exceed 3000mg APAP in a given day but i'll take 4 Norco 10/325's 1-2x per day regularly. Wondering if it would be safe to take possibly 6-7 Norco occasionally because I barely buzz off 40mg hydrocodone anymore.
 
Also I've read cimetidine (Tagamet) has been shown to inhibit the toxic effects of APAP to an extent, even significantly in rats. Wondering if this holds true to humans, and how funny a drug used to potentiate many drugs also having protective traits to it.
 
Antioxidants in general protect the liver from APAP damage.

The main active compound in milk thistle is used in it's pure form to combat APAP overdose damage.
 
My answer would depend on how often you have beem taking high doses, and for how long you plan on taking 7-8 per day. Why not do a CWE?

Too much apap over time can really damage your liver. Are you taking it for purposes where the APAP is beneficial at all?


I am personally not a fan of ever consuming APAP. I have had more than enough for this lifetime. Also, all the meds I know with APAP are not effective for me at 'reasonable' doses. If vicodin or percocef is all I have, and since in the last few years I have steered clear, I would probably nof hesitate to take 4000 mg in a day... not recommended, and not for any consecutive days.
 
Well I've got2 bulged disks putting pressure on my sciatic nerve so I take the norco for pan relief BUT also to get high. The high takes my mind off the extreme pain.

I've been taking 4 Norco 10/325's per time 1-2x a day for the past 3-4 months with a couple days breaks here and there. I never exceed 4g a day, usually 2600mg/day over 2 doses is what I max out at.

I had been taking milk thistle up until recently (ran out) and I also usually take cimetidine to potentiate which as stated above has protective properties against APAP induced hepatoxicity. Also take multivitamins and Vitamin E supplements and drink mostly water, and never alcohol.


But I would like to occasionally nod out on hydro by taking 6-7 Norco without having to go through trouble of CWE as having the gf wonder what I'm up to, you know?
 
Personally, I don't exceed 2600mg of apap, so anything more than 8 norcos or percocets and I will do a CWE. If I was using these more often, that number would go down.
 
Actually, any amount of paracetamol is bad for your liver. Taking 500mg every day is quite damaging. Taking 2400mg500mg a day is very harmful. 4+ grams and you are in immediate danger.

Basically, tylenol would never pass an fda review if it was introduced now, but it is grandfathered in. Remember vioxx and celebrex (which sickened and killed a lot of people)? Same family of drugs, same pathways.

If possible, get a pure opiate analgesic, or investigate compounded forms of hydrocodone.
 
I'd say it all depends on the individual. Like everyone has said, its definitely unhealthy but I have taken 6-7 norcos at a time without CWE and been fine. On the regular I've taken between 3-4000 mgs of APAP a day for an extended time with no ill effects also. This is something I hope to move away from soon, just trying to find the best option. Really need to perfect CWE but I'm having problems and there are wayyyy too many different opinions on how to do it.

It has nothing to do with how "we feel about it" - but instead, as others have pointed out, that there is a medically safe dosing ceiling (0g - no, I kid, but still).
 
You should be doing a CWE, you are putting a fair bit of stress your liver for no reason. Taking APAP daily is a bad idea.

They are so simple and it would take you maybe 10 minutes from start to finish!

There is no reason for you not too!
 
Well, a CWE that's done carefully is going to take more than 10 minutes, however even taking an hour or two it's worth the time to save time later (aka not being in a hospital).

Taking 2600mg a day is very dangerous. The 4000mg "limit" is for a one-time-thing type deal. Don't think you can take 3999mg of APAP a day and be just fine.

If you have to take that much daily, which some people do and there's nothing wrong with...either spend the short amount of time doing a CWE or switch to a pure opiate preparation...
 
I use to take up to 25 - 5/500 vics per day which is 12,500mg daily for several months at a time. You'll be fine if you can stop for long enough periods at a time to allow your liver to recover. If you take a high dose of APAP for years straight though you will almost surely get liver cancer or the like.
 
Oh I know, I highly recommend NOT doing that. I was just saying I use to do it cause I was naive that high doses of apap were so dangerous. Now I would never take that much.
 
10g is the LD50 (as mentioned by another member in a different, but nearly identical thread [as NT said, this is asked / answered multiple times per day / week]), meaning that suggesting 12.5g is even remotely non-life-threatening, is dangerous and ignorant.

Don't risk the lives of others just because you've been lucky enough to live through russian roulette. Chances are your liver is fcked already, taking that much APAP in... Just because it doesn't hurt, doesn't mean a thing.

Any amount of daily APAP use is dangerous.
10,000mg a day (10g a day) can kill some people. 4,000mg a day (4g a day) is an emergency situation. If you have to take APAP daily, please, please speak to your doctor about switching to a different pain medication or do a CWE. CWEs are very easy to do and you've only got one liver.
