Opioids Moclobemide with hydromorphone

I'm currently taking 450 mg of moclobemide daily. 150 mg with breakfast, lunch, and supper. I'm getting some hydromorphone tomorrow and I was wondering if it's safe to take the hydromorphone while I'm on the moclobemide. Should I start low with the hydromorphone and increase the dose slowly if there are no negative effects? Or is this a combination that should be avoided entirely?

Avoid it entirely. It is NOT safe to mix with hydromorphone, it makes opioids MUCH stronger.

The combination of pretty much any drug with an MAOI should be avoided. Not only will the effects be increased a shit ton-fold, but they will be extremely prolonged. A potent opiate like hydromorphone and moclobemide is a dangerous combination.

I'm guessing you'll probably do it anyway though. If you do, start very very small. But I don't pretend to know the extent of which the two will interact, and I do not an any way recommend this.

I would not dose it at all with an MAOi. Not worth the death risk.

can someone tell me why? i never heard of this before. i thought it only applied to tramadol.

i've taken selegiline with tilidine, dhc and hydromorphone without any ill effects...

It's contra-indicated in use with opioids, it is an MAOi, which you should know having taken them, are not to be fucked with.
