Opioids Methadone Dose Questions?

Hey all.
New here. Well been browsing here for some time now, but this will be my first post.

I am currently on a high daily dose of Oxycontin. About 600-700 mg's daily.
Question is, What would be a proper starting dose of methadone and how often should it be repeated?

Should I wait a certain amount of time after last dose of oxy before taking Methadone?

Can anyone recommend a good methadone taper regimen?


Really appreciate the advice, I am getting ready to enter a drug treatment program in a couple weeks but would like to make the transition as easy as possible.

You should start off with 30mg. Initially this dose may not hold you for 24 hours, so you might end up re-dosing later on (keep in mind methadone takes a long time to kick in, up to two hours or more), though I would take only an extra 5-10mg. You have to keep in mind, when doing a methadone taper, is that due to its ~24 hour half life, even if you wound up taking 20mg on day 2, you would be adding that to the 15mg left from day 1, so you would,, at a certain point, have a higher concentration of methadone in your system on the second day-even if you took less than day 1's dose.

As for having to wait to take the methadone-well, the good thing about old 'done is that you technically dont need to wait, but you really should if you have recently consumed any other CNS depressant. I would wait until you really feel in withdrawal to take it, as it will work quicker and you'll be able to gauge how effective your starting dose was in relation to your tolerance.
