IV'ing Codeine

We know that it is not good for you and you should not IV codeine of any formulation, mostly due to a great release of histamines. I was curious, if you were to simply take a moderate-high diphenhydramine dose prior to any codeine injection IV would that resolve most of the threat that IV'ing codeine entails?
 
Hellos mate, would still suggest that you do not iv codeine, we know it's not good and I don't think it's worth the risk really, someone with better knowledge will come along soon and answer your question I'm sure, but please don't iv it like I said it's not worth the risk :)
 
No, don't risk it. Put it up your arse or something instead. I'm not sure exactly on the pharma side of histamines, but I know from experience that for me it often takes dozens of antihistamines over several hours to quell my itch. IV'd, I do.ubt a small amount of oral anithist will have much effect; it's designed for when you're feeling a bit sick, not to stop an immune response to IV drugs.
 
IV codeine can sometimes cause life threatening pulmonary oedema (fluid in the lungs) from the massive histamine release. It can kill you - don't do it!

edit: it also can cause a life-threatening drop in blood pressure, exacerbated by the histamine release causing your capiliaries to become leaky and fluid to move into the wrong place - out of the veins and into the tissues (this is why you swell up when you have an allergic reaction, and is the cause of anaphylactic shock). In fact this could be termed anaphylactic shock, as the mechanism is identical.

The latter is actually more common as the cause of death from IVing codeine than pulmonary oedema I believe. Either way - it is very risky.

I am not considering IV'ing codeine nor would I ever. However say you could be IV'd an anti-histamine at the proper amount to stop any and all histamine activity from codeine, would this be stopping all of codeine's negative effects?

My question is really whether codeine causes more harm then that due from the histamine release, does it do any other damage any other way?
 
I don't think an antihistamine would be sufficient to stop a severe anaphylactic-type reaction, as the treatment for that is IM adrenaline - an antihistamine isn't enough, so I don't think an IV'd antihistamine would make it 100% safe (I appreciate you're not intending to do it, just discussing it hypothetically :) )

I don't know about any other specific dangers apart from the histamine release - that's not to say there aren't any, but I can't think of anything myself..
 
I think you're right, hehe. I'll leave it open a bit longer in case anyone else has anythig to offer however, the thread's only been here for a day or so, you never know :) I'll try and look into it in a bit more depth too.
 
Codeine has no effects in of itself, it has to be processed by the liver into active ingredients first.
This means that injecting codeine won't give you any effects, except bad ones, and the only way to take codeine is orally.

Besides inject it an you'll probably die.

Why would anyone want to inject codeine.
 
^ injected codeine will still reach the liver, it just doesn't have a direct path there like it would from the stomach (which is called first pass metabolism, as it passes through and is metabolised in the liver first). All your blood passed through your liver at some point. So yes it would be metabolised and it would work, but it would also likely give you a massive histamine release which could potentially cause life threatening anyphylactic shock or pulmonary oedema.
 
There is codeine injectable tablets with the only inactives are lactose and sucrose, they still need a micron filter to make them sterile for IM/SC and recommend against IV due to hypotension(from histamine) and seizures. Codeine IM/SC kicks in 10-30 minutes vs 1-2 hours orally. I think I read codeine is twice as strong injected compared to orally, not sure. I doubt an antihistamine would counter the massive histamine release, probably need something like epinephrine, which won't be pleasant.

And Howard Hughes was addicted to IM valium and codeine, don't think he IV the codeine. Why a man as powerful and rich as Howard Hughes used codeine and valium when he could get anything, I don't know, he was OCD.

If you must inject codeine use a micron filter and do it SC/IM.
 
^indeed, to clarify adrenaline=epinephrine, I was just using the UK not the intenrational phrase cos I'm lazy ;)

It's one of those things where people have done it and have been okay, but just because one person doesn't get a fatal histamine release/pulmonary oedema/anaphylactic shock-type reaction doesn't mean you won't either. On the scale of risky things to IV, codeine is pretty high up on the scale I would say.

I agree that IM/SC has less chance of a huge generalised histamine release but IM/SCing a pill is very risky as you're gonna get a lot of crap sitting there not being carried away by the blood. You may well get a huge local histamine response too which would be pretty unpleasant and severe swelling can sometimes even cut off the blood supply or squash the nerve supply in the area. If you are going to do either, IM is vastly safer than SC in terms of abscess formation as the muscles have a pretty rich blood supply, but then again if you are trying to get it absorbed slowly then IM might still be too fast..

Agreed that there are far superior things than codeine and diazepam out there ;)

And yes please micron filter anything you inject, especially pills! Overall though I really don't think injecting codeine in any way, shape or form is a good idea. At all.
