How long after fent IV do u have to wait for sub? (is it the standard 24? I kno it varies by person but ballpark me...)
What about how long to wait of u put the patch in ur mouth? (starting time "last dose" being whenever u take patch out of mouth)
 
It's always suggested to wait at least 24 hours since your last full agonist before dosing buprenorphine. However, IV fentanyl has such a short duration and half life, that you can probably get away with taking the bupe sooner than if you were using say, heroin or morphine.

However I would still try to wait as long as possible. How much fentanyl have you been using?
 
I had heard from my sub doctor that fentanyl is the one opioid that can bind stronger than buprenorphine to the opioid receptors.

I believe you don't have to wait at all, because the reduction in effects will be minimal given fentanyl's strong affinity for mu.


edit: realized you are going FROM bupe TO fent... nevermind. well IME i took full agonists and 6 hours later took sub with no precip w/ds. if i took sub 4 hours after last agonist dose i would experience approximately 45-60 minutes of precipitated withdrawals, but after 45-60 mins they went away and the bupe took effect. it's not like the bupe won't work, it just takes about an hour and gives you some temporary WD

The best way to figure out for you is to use the COWS (clinical-opioid withdrawal scale).
 
If my last dose was sublingual then what's he half-life and is there still some fent being released (I know transdermal continues releasing after u take te patch off thus 24 hours doesn't start when u take it off for sub dosing) ??

I know IV fent is dangerous but I figured out max dose possible (if I extractes 100% of the patch) per x Units of water real carful and started low. It worked out. :) I was HR conscious :)
