Opioids info on an Opiate Conversion Chart?

Hi again,

Tried searching the forum but found bupkiss. I was wondering if anyone knew of any online resources that will provide a formula or chart for various opiate conversions. I talked to a couple pharmacists and they said it should be available online, but I am unable to find anything of the sort.

Specifically, I'm wondering what a 25mcg fentanyl patch would equal in terms of 8mg dilaudid tablets or 10mg oxycodone tablets.

I just want to know what to expect as I am currently going off the patches, and my doctor hasn't really let on as if the knowledge of conversion is some sort of secret.
