I want to share with you some very important news regarding immunoassay urine testing for oxycodone.
I am a chronic pain patient that regularly sees a Pain Clinic which preforms random U/As on their patients. I seem to get them all the time being in my twenties. Several weeks ago, at an appointment, I was informed that my las UA was NEG for oxycodone. I was shocked because I took the drug literally 12 hours prior to the test. Due to the contractual agreement I signed this was ground for dismissal. I can only assume they believed I was diverting my Oxycodone/APAP.
I was determined to find a cause for a false negative as I trusted the lab completely. As it turns out, the AACC or American Association of Clinical Chemistry has repeatedly addressed this problem. Individuals taking below ~40mgs of oxycodone daily may often FAIL to meet the standard 300 ng/ml cutoff for immunoassay for oxycodone urine testing. I was shocked. How could a pain management clinic not be aware of this? Simple... most of their patients take more than 50mgs of oxycodone daily. After some rather heated discussion, i presented the AACC's report along with a similar report from the Administration on Veterans Affairs regarding this phenomenon.
Should you be a pain patient on lower dose oxy treatment receive a false negative result on a U/A. DEMAND a GS/MS test to be preformed on the sample in lew of a "one-step" urine sample, immunoassay. A brief google search of "flase negative oxycodone" reveals hundreds of similar reports with many individuals, in need of care for pain, being discharged from programs due to ignorance. If you take <40mgs of oxycodone daily (particularly in the IR forms and having a low body fat content) your readings are likely to fall in the 200 - 250 ng/ml range... just below the standard "one-step" cut off...
