I echo others' thoughts. Get a new doc, you really shouldn't have to feel neglected by him. Do you not take any NSAID's at all? I found that Naproxen/Diclofenac worked better than any opium for my slipped disc. Also pregabalin was brilliant for nerve pain, very effective. You really do have a lot of options and if he's not going to open the doors to them, then you have to do it. As somebody else has said, go see a pain management specialist.
I can totally relate to you not being able to sleep. Apart from kidney stones, my slipped disc is the only thing that brought me to tears and I could never sleep. Fortunately I was prescribed Zopiclone which was a massive help for me personally. If I was told to take Benadryl... well. I don't even know what thoughts would be crossing my mind!
Be proactive and find another professional who is prepared to look after you.
