Bupe Hydromorphone with buprenorphine

If you are needing that much hydromorphone to deal with your pain, there is no possible way that any amount of bupe will provide that much relief.

To me it sounds like they are trying to taper you off. I can't think of any other reason that they would prescribe that.

I have never heard of any doctor doing this, but again I'm not a doctor.

A doctor put my friend on the bupe patch along with hydro so it does happen. I dont think alot of doctors know how bupe works actually

I think you are right about the doctor not understanding the pharmacology involved when taking buprenorphrine and hydromorphone together. They were very insistent that it would provide adequate pain relief, wouldn't effect the hydromorphone analgesia and it being safer from accidental overdose being on buprenorphrine and reducing the hydromorphone dose by even 4 mg per day.

For some reason the extended release hydromorphone is more worrisome to them than the IR, that's why they upped the IR dose and completely took me off of the extended release and replaced it with bupe. I don't think they understand the buprenorphrine will completely block they IR. They just believe it will take away the opiod "high" but will still provide pain relief.
 
I'm not as worried about getting cut off completely as I am about the buprenorphrine causing side effects such as increasing pain levels or withdrawals from the hydromorphone. I'm fairly certain that after trying the buprenorphrine for a week if it doesn't help they will discontinue it and I'll just be 4mg off from where I started and I can get by with cannabis edibles filling in the gap.
If it eases your anxiety a little, its pretty easy to find your own solutions worst comes to worst. I mean Kratom is cheap, relatively effective (though depends on the person) and is really easy to get and is pretty safe (relative to other opioids). You can buy it on the clearnet easy peesy.

You can even buy proper opioids on the darknet though legit rx meds are usually pretty expensive. There are cheaper options that aren't as safe like heroin/rcs/pressed pills etc but uh, not recommended... this is HR afterall. There are some cheaper rx opioids but they are usually the atypical ones like tramadol/tapentadol. Those might not cut it for a dilaudid dependence....

There's also poppy seed tea. I used to get high on poppy seeds all the time. Just fill a 2L pop bottle half full of seeds, fill the rest with warm but NOT HOT water. If you have a strong stomach you can add a little vinegar help get the alkaloids out but it will make it taste much worse. Then shake intermittently for 20 minutes or so. If the solution becomes white and cloudy, you've kept them in too long, dump it out. You want it looking greyish. Its gonna taste real bad, but should help the pain. (EDIT: Just incase it wasn't clear, don't actually drink the seeds, you will get sick af, you just want the alkaloids on the outside of the seed)

Buuuuut kratom's probably easier though. You just have to mix with water and drink. Also tastes bad, but not that bad, kinda like green tea sludge if you ask me.

If it doesn't work and I take the patch off, how much longer will it stay in my system and block the hydromorphone? I want to atleast give it a try so I can tell them I tried it. I also have a calculus test this next week and I fucking hate math so I don't want to bomb it because I'm not sleeping, am in pain and cant think.

Uh hard to say. its not binary.. depends on the dose/person etc. You might feel something after 12 hours. 2 days should be enough to feel most of the effects though there's a chance it will be dulled a bit still. Thats what it was like with me when I started on pills. But now that I've been on them for 10 years, I seem to only have to wait an hour or two. I think at this point even 16mg doesn't fully saturate my receptors anymore.

This provider seems to be under the impression that buprenorphine is some kind of long lasting, low risk miracle drug for chronic pain.

I've also had consistent problems getting the hydromorphone er filled due to shortages and it's a pain in the ass for them when they have to get the prior authorization with insurance every few months because it always gets denied initially and they have to appeal it.
