Opioids Hydromorphone IV Dosage after Break from Opiates

Hey - I'm coming off six months clean of various opiates - I've been chipping here and there for the last 2-3 weeks (primarily Oxycodone IV 10-20mg dose maybe 7 times - The tolerance drop is amazing). Here's my question - I used to do LOTS of Hydromorphone and recently came into posession of some 4mg Dilaudids. I used to do Hydromorphone quite often - more so than Oxy, H, whatever - Typically 12-16mg IV shots. Given the fact that my Oxy tolerance has dropped so much and I expect my Dilaudid tolerance to drop as well - what would be a "safe" IV dose of Dilaudid - will my Dilaudid tolerance have dropped at the same rate as my Oxy tolerance? Is a straight Mg-Mg conversion in dose from IV Oyxcodone to IV Hydromorphone a good idea?Any ideas would be appreciated.

i'd say that if 20mg of oxy gets you good than 6 or 8mg of dilaudid will feel great

When you say 6-8mg are u referring to IV Dilaudid? When I use an opiate converter 6-8 MHD of IV Dilaudid equals 60mg IV Oxy which would knock me on my ass.

^ I definitely would not take 6-8mg of hydromorphone IV with six months of sobriety. Maybe 2-4mg instead.
