Opioids How to treat a tramadol overdose?

Hi Bluelighters.

I was just thinking that if in case of a possible overdose of tramadol what are the most effective medications to reverse its effects. Many of you know that tramadol is an atypical opioid that causes seizures and I have read that naloxone increase seizures from tramadol, so that doesn't seem like a good idea
I was thinking that maybe a benzo would be ideal.

Benzos, but they add some sedation, it's complicated. I have had several overdoses of tramadol, the truth is that they have not treated me with any specific medication. Maintenance only. Be careful with tramadol, it is so dangerous, my undoing is on it. Stopping taking it makes me a crybaby and taking it again makes me very compulsive and stupid, 2gr is my limit, after that, I can't remember anything else.

I took almost the same, 1.6 grams and without any benzo. I wonder when my luck ran out and I'll end up with a nasty seizure. Luckily now I was able to get myself a few benzos.
I thought that to overdose on tramadol it was necessary to mix it with other depressant drugs. At least with tramadol alone you need to take stupidly gigantic doses. And almost all documented cases are of suicide attempts.
They didn't even give you a benzo IV? I was under the impression that tramadol did not cause as slow breathing as other opioids. How did you feel prior to the overdose?
 
It’s complicated which makes it even more dangerous when you get to stupidly high doses like that. I’ve seen a lot of drs just observing and managing the patients condition.
I definitely wouldn’t be adding a benzo on top of a large dose of tramadol on my own. In a hospital setting if it’s what they use then fine but it’s just too risky. If you’re afraid of seizures then honestly you’re gonna have to reduce your dose. I personally know 2 people who’ve had seizures due to tramadol and it wasn’t pretty. One has a permanent tic now that has caused him all sorts of issues. Though they could be dead so I guess they got lucky.

@Nicocastillo7 Honestly dude, 2g! That’s soo high. Please be careful, if you can taper down a bit at least. You really are just taking a chance now every time you take those high doses.
 
Please do taper down, two grams of tramadol is insane and it's only a matter of time before you suffer seizures at that kind of dose.

I echo @PrincessDiz in not recommending benzos. They do treat seizures, however they will also increase respiratory depression. So you are reducing one risk but increasing another, especially when you're taking 2g, and benzos + high doses of opioids often ends with death.

I am not taking tramadol at the moment, my last takes have been 750mg or 600mg, no more than that, I am trying to get rid of this bad habit from my life. Today is day 3 off tramadol. I only mentioned 2gr because it was the highest dose I have taken, in that dose I lost consciousness and then I woke up the next morning without remembering anything and with pain in my muscles (I guess I have convulsed) I am miraculously alive, The times that I have taken more than 1gr of tramadol have not been few, I had that habit a while ago. That is why I have resorted to other opiates such as oxycodone, more potency without so much risk ... except going into debt to pay expensive boxes. What I could tell anyone new to tramadol is that if you no longer get anything from 400mg, just try to quit, it becomes a mess when you take more than is "safe" and see nothing wrong, since it will eventually pass
I have never taken tramadol without benzos, but that doesn't take away the risk

How did I feel before the overdose? I'm not going to lie, I was in an orgy, I was in heaven, eventually I had a blackout and everyone else did too. But I have been hospitalized for other overdoses and have only been monitored and given IV benzos, and other things that I have no idea



There have been no suicide attempts in any of the cases

Like in an orgy literally? Wow. Tramadol almost always kills my libido and I can hardly cum haha. :LOL:
 
Tramadol is the only opioid I'd never get down with. I don't even entirely know the benefit of giving an opioid a potential seizure aspect? There must be a positive quality to that side of the medication.

Worrying about not being able to breathe from painkillers is scary enough.

Well when it was approved by the FDA it was thought have really low abuse potential but they based this on research whereby tramadol was injected. There was research showing that high oral doses given to opioid addicts produced opiate like effects but of course the FDA ignored this. They even placed in the same category as viagra I believe when it came out.

You'd think after the 8th time of releasing a new "non-abusive" opiate would make them inevitably realize that there's no such thing lol! They'll never learn though imo.

I would bet that the FDA did not research the different forms of oral administration of tramadol either, since I feel that in oral drops the effect of tramadol is much faster than in pills or tablets. With an empty stomach, at 15 minutes I already begin to notice the effects.
 
2 grams damn... The highest dose I ever took (in a 12 hour span) was 750mgs. I've never seized luckily, but have seen a friend have one after a 500mg dose and he got rushed to hospital. To top that off we were in rehab and using tramadol to get high and still pass the opiate dip test. Which means we got caught (he ended up having an "honesty" moment and snitching me out). Asshole program director cut off my subs to force me to confess and it took another month before I did. He even rubbed salt into the wound after I confessed by refusing me hospital detox, and "home detox"ing me with 30mg diazepam a day wtf. Oops ADHD rant.

Overdosing? Complicated issue. Benzos (esp. Clonazepam) fight off seizures, but then again, they also potentiate tramadol.

Wow I'm usually a "know it all" but when it comes to this question, I really don't know...

I'm still a bit confused as I was under the impression that tramadol did not depress breathing as much as other opioids, so I don't see why a benzo would be such a bad idea. Of course, I am talking about treating an overdose patient in a hospital setting.

Did your friend have any tolerance to tramadol? Or had he only taken it a few times before and decided that day to take 500 mg? I have found that there appears to be a certain tolerance to seizures in dependent tramadol users, at least at doses less than 800 mg. Also 500 mg. in one dose is much riskier than taking it in smaller doses.
 
I'm asthmatic and tramadol + alprazolam sometimes depressed my breathing. Not majorly though; just a bit.

We both had no tolerance; I had abused tramadol many times before though, and he had never tried it. He also drank a bottle of DXM syrup and has a history of seizures. It was funny cause he looked it up and even mentioned the "dangers or seizure" before we did it, then had one.

That's the problem. People with a history of seizures should avoid tramadol or take it in small doses. Now is your friend okay or is he left with a sequel?
