Opioids how to lower tolerance to hydromorphone

I started taking oxy IR and ER pills for chronic pain abouta year ago. Drs moved me to Hydromorphone IR`s and ER`s (capsules here inCanada).
I take 80mg oxy ER in the morning just to get outta bed. By mid-afternoonI am hooping Hydromorphone and lots of it. By days end I have hooped 150 to180mg of Hydromorphone. (then I take another 40mg of oxy ER for sleeping pain)I hoop 30mg at a time. Sometimes I crush up the IRs or sometimes I crush up capsules.Anyway my tolerance is way outta control and I need to get it down. Does anyonehave any helpful ways or ideas of lowering my tolerance? I know I cannot go onlike this.
 
unfortunately the only way to do that is to lower your dosages to the point of almost withdrawing, sort of like hitting the reset button. you can try a potentiator but i doubt that will do much.

Calcium channel blockers (pregabalin/gabapentin) and NMDAR antagonists (orphenadrine, memantine, ketamine etc) are helpful medicines to prevent tolerance buildup to opioids when used for pain management. Strong NMDAR antagonists are particularly helpful for getting tolerance down.

You could ask your doctor for a script for memantine or orphenadrine if you can put up with the side effects. It might help with the tolerance issue.

I thought pregabalin/gabapentin were helpful in withdrawl. anyway I might ask dr for memantine or orphenadrine if that would help. thanks for the info!
 
you will probably be better off actually going into a withdrawal for a day or two

the last time I did that it worked but it was at a much lower dose. well it worked a little. I got it down now to 120mg of HYdromorphonre vs 180mg daily. the oxy amounts are the same.

Or cut your dose in half for a bit.

Also try benadryl as a potentiator.

Doesn't matter what you use it for, if you want to lower your tolerance in any significant way, that is what you have to do.

Using it for whatever reason doesn't change what the drug is.

Plus at that dosage you're going to have A LOT of pain when you stop if you actually have "real pain" still or not.

Don't like it? Don't lower your tolerance then.

Well, you should cut your dose by 25% for a few days, 50% for a few more, go without and withdrawal for a few, and then you can go back to a slightly lower dosage than you take now while getting the same effect... For awhile.

Opiate tolerance is problematic and there's no real "good" way to deal with it.

As I said, potentiation will help to (try to) reach desired effects with lower dose.

already been doing that, down from 180mg hydromorphone to 120 for the last 3 days, to hopfully 90mg today.
