Opioids How to gauge morphine xr dose......

I took 80mg of hydrocodone last week after being on 1mg subs a day for 2 months and felt absolutely nothing (was off subs 2 weeks). I will have 8 extended release morphines later, 40mg a piece.... no sure how many to take or the equivalency especially since they are xr. Any input is appreciated.
 
Check the opioid conversion chart here at BL.. I don't know how to link to it on mobile. Then work from there. Maybe crush the pills and parachute. I assume you're dosing oral?
 
I wanted to plug them but have no clue how to...and no clue because of the xr formulation.... i can find a conversion, but again, this is the xr formula so... I'm really stuck
 
It's still the same chemical. There's no special conversion from IR morphine sulphate, other than trying to defeat the ER formulation. 80 mg seems like about 120-150mg plugged MS. As for plugging, use an oral syringe and dilute the powder as much as possible. Basically IV process minus filtering necessity.

If you search on here, I have written a guide on how to turn morphine ER tablets into liquid morphine solution for rectal administration (I get 18+ hours of pain relief out of a few hundred milligrams of rectal morphine, when it would take like... 500-750mg orally for me to produce the same pain killing effects.

As for how to gauge your dose, depends on your tolerance but here's the thing. Morphine isn't like hydrocodone or buprenorphine at all, its WAYYY stronger especially compared to the doses of hydrcodone and buprenorphine you were taking, it's a whole different ballgame. Morphine is essentially heroin. Heroin turns into morphine into the body upon admninistration as heroin itself is inactive, it just gets the morphine across the BBB much faster


I have never heard of 40mg morphine tablets being made, what brand are they / where are you from, generally? like, USA? Europe? I'm pretty well versed with all the morphines as it has been my DOC for about ten years but I pretty much only mess with the [M]200mg, [M]100mg ER, mallinckrodt ER tabs and the 15 and 30mg IR tabs or Roxanol.
 
I'm sorry but heroin is still considered to be 8 times stronger than morphine even despite that it's practically a prodrug for the latter. And it's certainly not more potent than buprenorphine... a 30mg morphine being roughly equivalent to 32mg bupe? Hell no. I respect your opinions on this site but it seems to me you're giving potentially dangerous impressions.

40mg of Morphine XR isn't that strong. Because its extended release. I usually take 30mg of Oxy and 200mg of Morphine XR. the 200mg XR feels slightly stronger with a longer half life. So I would try two of them for starters. But 120mg wouldn't be pushing it since you take 80mg of Hydro.

This depends greatly on the ROA. Injected, heroin is ~2.2x morphine, orally it is essentially a prodrug with ~90% BA for morphine. (so, about 3x morphine, which has an approx oral BA of 30pc.)

Apparently KADIAN has a 40mg formulation of XR morphine sulphate. (MS Contin doesn't) here's the prescribing information -
Patients receiving other oral morphine formulations may be converted to KADIAN by administering one-half of the patient's total daily oral morphine dose as KADIAN twice daily or by administering the total daily oral morphine dose as KADIAN once daily. There are no data to support the efficacy or safety of prescribing KADIAN more frequently than every 12 hours.

KADIAN is not bioequivalent to other extended-release morphine preparations. Conversion from KADIAN to the same total daily dose of another extended-release morphine product may lead to either excessive sedation at peak or inadequate analgesia at trough. Therefore, monitor patients closely when initiating KADIAN therapy and adjust the dosage of KADIAN as needed.

So I'd hazard a guess then that when taken normally (orally) it's about twice as long and a little more than half as strong as comparable oral morphine. that is, 40mg Kadian = 20mg morphine followed by another 20mg 6 hrs later.

You could probably crush the beads or something to make it IR.
 
Ya the kadian beads are a little bigger and easier to work with than the xr adderall beads
