Opioids How much methadone to block the pain?

I feel bad for posting a new thread just for a quick question but I looked all through the methadone megathread and other threads I found using the search engine and couldnt really find an answer to my problem....so please dont bash me for posting this....


I am currently taking approximately an entire one and a half of the 100mcg fentanyl patches a day.....meaning, I put the patch inside my mouth on my cheek for a couple hours until all the fentanyl has absorbed into my body or gotten swallowed. And then I do that again, but with only half a patch. I have been doing this every day for a few months, as well as taking 8mg dilaudid pills every four hours.
So, needless to say, I have a massive tolerance to opiates. I take these drugs legally for a spinal problem I have, but I was also an opiate addict before my spinal issue.

Anyway, my question is....I am going to be out of my patches early and have traded a few tramadol for 80mg's of methadone. I am wondering if this is enough to keep me from being sick and in pain for atleast 12-16 hours? Or do I need more methadone since my tolerance is massive?....im worried because the patches I take and the dilaudid barely keep me from being sick and in pain as it is, so I really would be taking more if I had more.

Anyway, im gonna shut up now. Any help is appreciated. Thanks.
 
So you're consuming 15mg of fentanyl a day buccally?

I used this narcotic converter and it says that 7.5mg of IV fentanyl (equivalent to 15mg buccal factoring for 50% BA) is equivalent to 112.5mg of methadone dosed acutely not accounting for incomplete cross-tolerance.

80mg will probably hold you pretty well - just take into consideration that the methadone will stay in your system for more than 12-16 hours so if you're going to dose that soon after, be careful.

yea I know. I almost got a "hard on" when this guy said he wanted tramadol for methadone. And I just happened to have tramadol(which I never take because it doesnt do anything for me). he said the methadone makes him feel weird so he doesnt like it.

I forgot to ask.....doesnt grapefruit juice help make it stronger?....something to do with slowing the metabolizing blah blah blah....?

...cuz I got some ruby red in the fridge.
 
Effects of grapefruit juice on the pharmacokinetics of the enantiomers of methadone.

BACKGROUND AND OBJECTIVES:
Cytochrome P450 (CYP) 3A4 is the main CYP isozyme involved in methadone metabolism. We investigated the influence of grapefruit juice, which contains inhibitors of intestinal CYP3A, on the steady-state pharmacokinetics of methadone.

METHODS:
For 5 days, 8 patients undergoing methadone maintenance treatment received 200 mL water or grapefruit juice 30 minutes before and again together with their daily dose of methadone. Blood sampling for R-, S-, and R,S-methadone plasma determination was performed over a 24-hour period. CYP3A activity was determined by measuring the plasma 1'-hydroxymidazolam/midazolam ratio.

CONCLUSIONS:
Grapefruit juice administration is associated with a modest increase in methadone bioavailability, which is not expected to endanger patients. However, it cannot be excluded that a much stronger effect may occur in some patients, and thus grapefruit juice intake is not recommended during methadone maintenance treatment, in particular in patients initiating such a treatment.

im gonna go down a glass of juice cuz i just dissolved the 80mg's of methadone in warm water and drank it.
I was thinking of plugging it cuz i read a couple reports on here about how it works even better and faster that way but my back hurts too much, i doubt i could contour myself into the correct position to do that effectively....allthough I am not the kind of person whose afraid of plugging....or doesnt believe it works. I have successfully plugged suboxone, oxy, and something else I cant remember :)

anyone have experience with plugging methadone? and also, how long should i expect to wait for this methadone i just swallowed to kick in?
