Also interested in the O-DT happening up there, BD. Would you (or anyone else) say the narcotic effects are more powerful? I love my Tramadol but sometimes it can't kill all my aches and pains.
I have never tried tramadol itself, only the O-DT, which I like a lot. I'm afraid that tramadol's pain relief comes mostly (if not only) from its metabolite O-DT, and O-DT's further metabolites, so if tramadol doesn't cut it for you, then O-DT won't either. The advantage of O-DT is that it has practically no ceiling - with tramadol, because of its serotonin reuptake inhibitor (SRI) property, you run the risk of seizures and all kinds of shit, potentially fatal. O-DT is not an SRI, so there's no risk of serotonin-related dangerous side-effects. So that means you can go somewhat higher with O-DT and perhaps reach higher opioid-based pain relief.
I actually don't understand why in medicine, tramadol is used, and not O-DT.
Tramadol, as a substance, has no opioid activity. Only its metabolite O-DT has. Both are norepinephrine RI's, so in that department they're nearly identical. However, O-DT has no serotonin syndrome risk. So why not use O-DT for pain relief instead? By all logic, O-desmethyltramadol is a better and safer substance than its prodrug tramadol.
If you use tramadol for a legit pain condition, and it isn't powerful enough, perhaps it's something you could talk to your doctor about? Being an avid (ab)user of both codeine and O-DT, I'm pretty sure O-DT is a stronger opioid, but sometimes it feels like codeine is stronger at pain relief. So you could try codeine for your pain. As far as opioid go, codeine is pretty benign and safe. If it doesn't work either, unfortunately you'll have to go "higher" on the opioid ladder, which is inadvisable. Well, those are just my couple thoughts, simply ideas; and you should take them with a pile of salt, because I'm obviously not a doctor. Which is why you should discuss it with your doctor first before trying new solutions.
OT: another 300 mg O-DT. Goddamn mens, I haven't felt this opiated and great in a looong while. This shit is what I've been missing for so long. Now I have to make sure I don't over-use it, because last time I did O-DT, I ran through 5 g in a week. Obviously, my opioidergic system in the CNS wasn't too happy about it and made me suffer some unpleasant WDs. Hope not to repeat the same mistake this time 'round.
